Essential Job Functions/Responsibilities:
Conduct assessments, treatment planning, evaluation of the response to care and readjustment of the treatment plan as needed
Plan and provide education to patients, families, and peers
Completes accurate and timely documentation of patient care treatments in EMR (i.e., HomecareHomeBase)
Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other organization professionals.

Service Coverage Area: Orange County

Position Qualifications:
CA Occupational Therapy License
2 years of physical therapy work experience
American Heart Association Basic Life Support (BLS) certification
Valid CA Drivers License
