We are excited to be hiring a RN Nurse Home Health Care Case Manager. The ideal candidate for this role will have the following skills and responsibilities:
ROLE RESPONSIBILITIES – what you’ll own and lead:
- Oversee nursing services and clinical standards, including the medication administration program, ensuring compliance and consistency
- Conduct Client nursing and medication evaluations, identifying risks and ensuring safe, high-quality care delivery
- Train Caregivers on policies, medication administration, and nurse-delegated tasks, verifying competency and readiness
- Maintain accurate and compliant clinical documentation in alignment with company protocols and state regulations
- Review physician orders, reports, and care plans to ensure accuracy, completeness, and regulatory compliance
- Collaborate with community providers and internal teams to coordinate care and support positive Client outcomes
- Ensure adherence to all in-home care licensing standards, regulatory requirements, and company policies
- Support continuous improvement of clinical practices, training, and care quality across the organization
EDUCATION AND EXPERIENCE
- Current RN license in the state of Oregon (required)
- Experience with EHR or care management platforms such as Epic, WellSky, AxisCare, or AlayaCare
- Previous experience in home care, community health, or case management preferred

- Demonstrate strong commitment to compliance
- Knowledge of best practices for handling PHI in compliance with HIPAA regulations
- Administrative specialty in related field – whether accounting, customer service, scheduling, billing, IT, project management, leadership, sales, marketing, business development, medical scheduling, staffing, care coordinator or coordination, recruiting, talent acquisition, compliance, office manager or management, business manager or management, human resources, or operations a bonus
- Experience in assisted living, facility, nursing home, or in home care a bonus
