About This Position
Job Summary: Under the general direction of the Department of Pharmacy, the Medication History Specialist is responsible for obtaining medication histories for admitted patients arriving through the Emergency Department. This will be accomplished through various methods including, but not limited to patient interviews, researching the patient’s electronic medical record, and contacting other resources to collect the most comprehensive and accurate patient home medication list as part of the admissions process. The Medication History Specialist will interact with and provide medication history and other related patient information to nurses, physicians, pharmacists and other medical professionals as appropriate. This role will have an impact on improving the safety and quality of our patients by providing physicians and physician extenders with a comprehensive home medication list. Although this role reports directly to the Department of Pharmacy, individuals working in this capacity will not engage in any of the dispensary functions of the Department of Pharmacy nor will they be cross trained within the Department of Pharmacy to work as Registered Pharmacy Technicians.
Responsibilities:
- Utilizes knowledge of medications and sources of medication data to accurately compile a comprehensive best possible medication history (BPMH) for patients admitted to Westchester Medical Center.
- Accurately enters a patient's medication history and pharmacy data into the electronic health record under the home medication section to be validated and verified by the provider during the medication reconciliation phase of the admission process.
- Interacts with and provides medication history and other related patient information to nurses, physicians, pharmacists and other medical professionals as appropriate.
Qualifications/Requirements:
Experience:
2 years of previous experience in a clinical setting

Education:
Required: High School Graduate or Equivalent
Preferred: Pharmacy Technician Certification
Licenses / Certifications:
Required (can be achieved on the job during the training period): Medication History Certificate from PTCB and ASHP
