As a member of our Transitional Residential Treatment Facility team, you'll help clients build life skills, manage their challenges and grow their personal strengths with the ultimate goal of successfully transitioning to greater wellness and independence.
- What you'll be making - Starting rate for this role is $27.14 per hour (Level 1 -- Direct Service), with a wage differential, based on regulation ($30.01 per hour for Level 1 - Direct Service, Regulated):
- Successful Candidate who are registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($34.41 per hour for non-regulated, and $37.26 per hour for regulated); for internal applicants, grid level placement will occur as per the Collective Agreement
- Premiums of $2.00 well be applied for eligible Evening/Night and Weekend Shifts
- When you'll work - 70 hours over a two-week period; Rotating shifts of Days, Afternoons, Evenings & Weekends
- Your Pension Plan - Immediately eligible for optional enrolment in the Healthcare of Ontario Pension Plan (HOOPP)
Primary Responsibilities:
Designated Primary Worker for assigned caseload:
- Ensure participant’s Electronic record and paper file is complete and up-to-date
- Ensure goals are being worked toward and program expectations are being met
- Provide participant updates at Team Meetings
- Create Team Approaches and/or Contract of Residency
- Follow through with primary responsibilities when a client’s Primary Worker is unavailable
- Assess and build client’s strengths, abilities, needs and preferences
- Assess social and environmental factors impacting the client
- Assist clients to develop and coordinate an Individual Rehabilitation Plan (IRP) based on their strengths, abilities, needs and preferences
- Review and revise IRP with client every four-six weeks, or more frequently if deemed necessary
- Motivate and support the client to work towards achievement of their IRP goals
- Include and encourage clients’ natural supports (e.g. family, significant friends) in the IRP, clarifying the roles each plays towards clients’ rehabilitation
- Completion of frequent Mental Health Status Exams within the first week of the client’s admission, and subsequently at a minimum interval of once per month
- Create or update client Crisis Plans
- Medication monitoring and education, and support clients with medication routine
- Completion of Medication Reconciliation procedure
- Ongoing liaison with RTF Nursing Staff Team to ensure a firm understanding of the clinical aspects of the RTF program
- Facilitate and implement resident meetings, small groups, and activities
- Provide health teaching about mental illness and symptom management
- Assist clients to find and maintain suitable housing
- Assist and coach clients in the learning and management/achievement of activities of daily living such as personal hygiene, daily chores, budgeting, socialization, etc.
- Provide recreational community outings and activities to enhance quality of life for clients and further connect participants to their community
- Collection and accurate documentation of monies for rent and groceries
- Completion of a Discharge Summary upon clients transition from RTF outlining progress made in the program and recommendations for the future
- Offer and provide follow up support to client in the community
- Minor maintenance of the facility such as repairs, common areas, grounds, resident rooms, etc.
- Professionally liaise with community workers and agencies regarding the RTF programs, referrals, intakes, and waitlists
WHAT YOU'LL NEED TO APPLY:
- Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 5 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
We Require the Following Qualifications:
- Degree of Diploma in the Social Sciences or a related Field (Primary proof of credentials will be required during the hiring process i.e. original degree/diploma or official transcript)
- Current, satisfactory Police Vulnerable Sector Check (PVSC)
- Valid Drivers licence and access to a vehicle with at least $2 million liability insurance to transport clients.
- Demonstrated ability to attend work regularly
We prefer the following qualifications:
- First Aid and CPR
- Applied Suicide Interventions Skills and Training (ASIST)
- Work experience in the addiction and/or mental health field preferred
- Experience working in a residential supportive housing environment preferred
- Knowledge of safe food handling procedures
- Knowledge of WHMIS, fire, health and safety procedures
- Knowledge of Psychosocial Rehabilitation
- Knowledge of psychiatric medications
- Knowledge of community resources
- Knowledge of computerized documentation methods
- Health and safety knowledge
- Crisis intervention and prevention
