LPN - ARHH at Ashtabula County Medical Center – Ashtabula, Ohio

Using the nursing process implements the plan of care established by the patient's RN PCM, in coordination with the patient's physician for Acute Care.

• Education, Knowledge, Skills and Abilities
  • Graduate of an approved School of Practical Nursing
• Required Length and Type of Experience
  • Recommend minimum of 2-3 years clinical experience in an acute care setting. Able to provide independent transportation.
• Required Licensure or Certification
  • Current Ohio LPN license and pharmacy card.
• Required Physical and Environmental Demands
  • Requires eye/hand coordination and manual dexterity sufficient to operate a telephone, analyze and prepare written records, and provide hands-on care to clients. Requires normal range of hearing to communicate with staff, clients, physicians, and other health care providers. Must be able to lift and carry 25 lbs. Traveling to patient's homes in a 60-mile radius in all kinds of weather. Must be able to access a wide variety of home structures. Involves dealing with frequent changes, stressful situations, and need to problem solve.
