INTEGRATED BEHAVIORAL HEALTH THERAPIST

Western NC Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC) dedicated to delivering high‑quality, patient‑centered, integrated care with purpose and integrity. We are seeking a compassionate and skilled Integrated Behavioral Health Therapist committed to clinical excellence and supporting the wellness of patients of all ages.
This role provides comprehensive behavioral health services within an integrated primary care environment, including assessment, treatment planning, crisis intervention, and solution-focused, evidence‑based therapeutic interventions. The therapist collaborates closely with primary care providers (PCPs), behavioral health staff, and the broader interdisciplinary team.

Key Responsibilities
Includes, but is not limited to the following:
Assessment, Diagnosis & Treatment Planning
  • Conduct clinical interviews and assessments, including Comprehensive Clinical Assessments (CCAs), standardized screenings, and biopsychosocial histories.
  • Identify psychological, emotional, or behavioral concerns and diagnose disorders using evidence-based guidelines and assessment data.
  • Develop, implement, and update individualized treatment plans outlining goals and objectives and determining therapeutic interventions, frequency, and duration of treatment.
Therapeutic Interventions
  • Provide outpatient therapy using solution‑focused and evidence‑based approaches.
  • Assist patients in gaining insight, defining goals, and building coping strategies to improve emotional, social, and functional well‑being.
  • Continually evaluate treatment effectiveness and adjust interventions or diagnoses as needed.
Crisis Intervention & Safety Support
  • Assess and de-escalate patients presenting in emotional or behavioral health crisis.
  • Collaborate with integrated care team members to coordinate crisis response and follow-up plans.
  • When necessary, assist with securing judicial orders for involuntary inpatient treatment.
Care Coordination & Integrated Support
  • Collaborate with PCPs, nurses, behavioral health staff, and care management to support integrated, patient-centered care.
  • Coordinate referrals to internal services (e.g., psychiatry, care management) and external specialty services (e.g., ACTT, IOP, IP) as appropriate.
  • Support patients in accessing public benefits and community resources essential for maintaining stability.
Patient & Caregiver Education
  • Educate patients and caregivers about treatment plans, mental health conditions, and the importance of medication adherence.
  • Teach and model effective coping strategies for managing stress and emotional challenges.
  • Communicate treatment options and progress with patients and, when applicable, guardians of minors.
Documentation & Professional Standards
  • Document all screenings, assessments, treatment plans, progress notes, and recommendations in the Electronic Health Record (EHR).
  • Maintain current knowledge of evidence‑based practices, clinical research, and treatment modalities.
  • Complete required trainings, participate in monthly peer review, and contribute to interdisciplinary team meetings.
Qualifications & Preferred Experience
  • Master’s degree in social work or clinical mental health counseling.
  • Fully licensed with LCSW or dually licensed with LCMHC and LCAS.
  • Experience providing behavioral health services in primary care, integrated care, and/or community health settings.
  • Experience functioning as a Behavioral Health Care Manager within the Psychiatric Collaborative Care Model (CoCM).
  • Experience completing clinical assessments, developing treatment plans, and delivering evidence‑based therapeutic interventions.
  • Ability to collaborate effectively within multidisciplinary and integrated care teams.
Skills
  • Skilled at working in fast‑paced environments and maintaining professionalism under pressure
  • Strong verbal and written communication skills, with the ability to interact professionally and compassionately with patients, families, and a diverse healthcare team.
  • Ability to plan, prioritize, and manage multiple responsibilities with high attention to detail.
  • Proficient in Microsoft Office applications (Teams, Word, Excel, Outlook).
  • Collaborative, team‑focused communication style with the ability to work effectively across clinical and community settings.
  • Working knowledge of medical office operations and familiarity with the role of Federally Qualified Health Centers (FQHCs) within the community.

Salary range: $65,000 to $70,000
