Care Coordinator at Linden Square Senior Care – Saline, Michigan
About This Position
Shift: 11a-7:30p
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Essential duties and responsibilities include the following.
1. Acts as contact person for Med Admin Program, passes medication, reorders medication, handles cycle medication, assists with medication destruction and completes Audits (Med Cart and Narc Count), and reviews MAR for conflicts.
2. Works closely with LPN daily and assists the nursing department on a daily basis with a variety of tasks.
3. Coordinates staff scheduling, including creating and maintaining the shift schedule, managing time-off requests, arranging coverage for absences, and ensuring adequate staffing levels at all times. Communicates schedule changes to staff promptly and fairly.
4. Serves as a contact person for the pharmacy.
5. Checks Med Room daily – ensures compliance including proper storage of med carts and policies being adhered to.
6. Coordinates medication administration training with HR, trains staff on floors with cares and daily routine. Monitors staff and retrains as necessary. Provides support and serves as a role model for staff.
7. Participates in “On Call Rotation.”
8. Available to rotate shifts as needed.
9. Completes daily cares, answers call lights as needed.
10. Orders nursing related supplies.
11. Watches for orders and scan to Nursing Supervisor.
12. Completes walk through of building at least 2 times/shift, checking all floors (if multiple floors) and verifying that all exit doors are closed tightly and locked.
13. Observes, charts on, and reports all issues of concern. Communicates changes in condition with the team.
14. Greets, assists, and directs visitors as needed.
15. All interactions with family members, residents, care managers, providers, and other staff must be done verbally, texting is not an acceptable means of communication.
16. Answers incoming calls and responds to inquiries in a positive and professional manner. Redirects calls, takes concise notes when necessary, and forwards proper recipient promptly. Listens to voicemails and answers appropriately.
17. Forwards resident and family concerns to the appropriate person if cannot find correct answer.
18. Attends, participates, and assists with special events/projects.
19. Attends and participates in all staff meetings and training sessions.
20. Participates as a member of the Quality Assurance Team.
21. Demonstrates the ability to function as a team member in sharing responsibilities for the administration of resident care in cooperation with all members of the team.
22. Ensures staff awareness of current policies and procedures, and ensures all policies and procedures are followed.
23. Refers to policy and state requirements to determine procedures to ensure requirements are being met.
24. Provides necessary support to ensure accountability and regulatory requirements of the company are met.
25. Upholds the high standards of resident care by monitoring and ensures the conditions of health, comfort, safety and well-being of residents, while providing and maintaining a clean resident care area.
26. Upholds and promotes the organization’s values and philosophy relating particularly to ethics, morality, and integrity as set out in Inspired Senior Healthcare’s Employee Handbook.
Requirements:- Must be able to relate to the elderly in a positive manner.
- Prior experience in a position within an assisted living or skilled nursing community.
- Must be able to easily lift and carry up to 50 lbs.
- Must be able to stand, walk, stoop and/or bend for periods of up to eight hours.
