About This Position
The LPN is responsible for the accurate evaluation of each patient’s status, problems and needs. The LPN collects and analyzes data from the patient’s medical history, nursing assessments and staff input on an ongoing basis. The LPN provides as needed; crisis care or visits as assigned and will provide coverage at the SFR inpatient units as needed. Schedule is every Friday, Saturday and Sunday- 8 pm - 8 am.
All care directly provided or coordinated by the LPN is in conformance with federal/state regulations. Shift differential paid according to payroll policy. Provides care for patients for the entire county.
REPORTS TO: Clinical Manager
DIRECTLY SUPERVISES: Hospice Aide services as needed
CERTIFICATIONS/LICENSES REQUIRED:Requires a valid, current Florida LPN license and a valid Florida driver’s license.
EXPERIENCE/SPECIFIC SKILLS AND ABILITIES REQUIRED:Minimum 1-year clinical experience, preferably in hospice, home health, oncology, acute care or public health. Ability to read, write and speak English. Bi-lingual preferred. Ability to manage time and phone calls received from patients, families, physicians and Triage RN on shift. Excellent communication and interpersonal skills.
PHYSICAL REQUIREMENTS:Ability to lift, push, pull twenty-five (25) pounds without difficulty. Good visual and audio acuity. Able to work under varying degrees of stress.
PRIMARY RESPONSIBILITIES:· As needed, crisis care and other visit types in patient’s place of residents, as assigned. May be required to cover at the SFR inpatient units.
- Collects and analyzes data from patient’s medical history, nursing assessments, and staff input on an ongoing basis.
- Keen clinical observation of patient’s condition, response to treatment/medications and recommend changes to patient’s Plan of Care.
- Coordinates care of patients with other SFR team members through case conferences, consultations with MD’s and community agencies.
- Prepares complete and accurate documentation in accordance with all SFR policies and procedures, Federal/State regulations and Medicare/Medicaid guidelines.
- Maintains strict confidentiality and respect for patient/family privacy and possessions.
- Practices Universal Precautions and safety at all times.
- Teaches patient/family skills needed to care for the patient to include personal care and medication management to provide/maintain maximum patient comfort. Provides emotional support to patients, family and other caregivers.
- Participates in performance improvement and quality control activities.
- Increases knowledge of palliative care through active participation in educational opportunities.
- Attends SFR General Staff Meetings, nurses’ meetings and educational training sessions as required.
