Job Title: Forensic Discharge Planner Case Manager III

Job Locations: 7460 Tidewater Drive, Norfolk, VA 23505

Schedule: Monday – Friday 8:30am – 5:00pm

Payrate: $24/hourly

Job Duties:

Serves as the agency liaison to the BRRJ and NCJ for the purpose of assessing individuals in the jail, to determine the presence of mental illness or co-occurring disorders. Individuals determined to have mental illness or co-occurring disorders will be linked to the appropriate NCSB program.

Physical Requirements of the Job:

While performing the duties oft his job, the employee may be required to visit the Norfolk City Jail, the regional jail and other criminal justice agencies. The employee will transport clients and make home visits as necessary. The employee may also be required to sit for extended periods of time, utilize a computer, operate various office equipment: copier, printer, facsimile machine.

Typical Working Conditions:

Works with a challenging population, often from diverse backgrounds with a variety of concomitant. Will spend the majority of time based in the city jail. Requires management of a number of competing work priorities and will require working a flexible schedule.

Examples of Duties:

• Serves as a liaison to the HRRJ to conduct face to face assessments of individuals in the jail to determine the presence of mental illness or co-occurring disorders.
• Works with appropriate jail official and pre-trail case manager to provide discharge planning/jail release for all eligible clients.
• Linkage to a mental health or substance provider in the community (CSB or private provider) that provides psychiatric treatment and/or substance abuse treatment.
• Provides case management to address short-term client needs which may impact recidivism and/or admission to a local inpatient facility.
• Assists client directly, which may include transportation, for the purpose of obtaining needed resources.
• Link with appropriate housing when possible.
• Conducts home visits and makes contact with family or significant others to promote community stability.
• Assures that client has supply of psychiatric medications upon jail release.
• Assists eligible clients who are Medicaid eligible, but not enrolled in Medicaid to complete Medicaid enrollment.
• Assist with obtaining birth certificate and photo ID.
• Completing social security disability application prior to release (completing the necessary paperwork and providing documentation to begin process of reinstatement of benefits).
• Link to a medical provider for treatment of any identified medical condition.
• Maintains client records in accordance with NCSB standards, State regulations, and Virginia statutes regarding jail release and discharge.
• Completes quarterly data reports on Forensic Discharge Planning activities.
• Develops and maintains a resource list of services for the target population and documents service gaps.
• Keeps outcome data and other statistical information on clients served.
• Attends scheduled staff meetings as required.

Knowledge:

• Mental health and substance abuse concepts and symptoms associated with multiple disability population.
• Assessment, diagnosis and rehabilitation for people with mental health and substance abuse disorders.
• DSM-V diagnostic criteria and psychotropic medications, side effects and uses.

• Crisis Intervention theory and intervention techniques
• Community Resources and how to access such resources
• Criminal Justice population and the agencies that serve them

Experience:

Must possess a bachelor's degree in human services, or related field, with extensive experience working with individuals with serious mental illness and substance abuse disorders and who have criminal justice issues and involvements.

Certificate/ License:

Possession of a valid driver's license and an acceptable driving record as reported by the Department of Motor Vehicles are required for persons using personal or agency vehicles in the performance of their duties.
