Job Summary

Treat actual or potential health problems using nursing diagnosis and case management. Teach, counsel and provide supportive care to the patient and their significant others

Essential Functions

• Performs a thorough and timely assessment of the patient’s condition, including physical, psychological, social and rehabilitative aspects.
• Identifies problems/nursing needs including discharge needs
• Intervenes appropriately to address identified problems and needs. Plans care according to standards of care. Manages time efficiently/sets priorities appropriately
• Evaluates effectiveness of care and revises plan if necessary
• Provides education consistent with the patient/family learning abilities and preferences; evaluates outcomes
• Effectively communicates with physician’s, co-workers, patients, families
• Reports pertinent observations regarding patients to physicians and other health care team members
• Shift report is given on time, accurately and concisely
• Documentation on the patient’s medical record is accurate, complete and concise
• Maintains National Patient Safety Goals
• Verifies, coordinates and carries out physicians orders accurately
• Coordinates interdepartmental transfers smoothly and efficiently
• Coordinates interdepartmental activities (i.e., lab, radiology, etc.) to ensure timely, efficient care
• Communicates and coordinates with Case Manager to ensure timely discharge, patient satisfaction and quality care along the continuum of care
• Practices Universal Standard Precautions
• Demonstrates knowledge of department safety procedures, including safe handling of equipment and hazardous substances

Minimum Education/Certifications
Graduate of an accredited program in nursing.

Current Registered Nurse license to practice in the state of NJ.

Certifications required: BLS (American Heart Association)

Minimum Work Experience
1 year or more experience in Behavioral Health preferred.

Position Type/Expected Hours of Work: Must be willing to work holidays, weekends and provide additional coverage as needed
