Community Health Worker at RWJBarnabas Health – Union City, New Jersey

Job Overview:

The Community Health Worker will primarily be working out in the community with patients to promote, motivate and improve healthy living. A CHW will assist patients in utilizing resources and accessing services to overcome barriers in order to meet their health goals.

Qualifications:

Required:

• High School Diploma or equivalent
• CHW Training/Orientation (completed while employed)

Scheduling Requirements:

• Day shift
• Full Time

Essential Functions:

• Work as a team with colleagues to share resources
• Schedule weekly follow-up calls and home visits to patients
• Make follow-up appointments with a Primary Care Physicians or Specialists
• Maintain external and internal documentation accurately via EPIC and Microsoft Excel
• Collaborate with clinical staff to discuss patient's health goals and barriers to optimum health
• Reply consistently and effectively to emails and secure chat messages from manager/ team and/or colleagues
• Assist patients with organizing their records- Assist patients in educating about MyChart, including signing patients up for MyChart
• Ensure patients who have Medicaid retain their coverage by contacting Medicaid, and/or help patients fill out their re-enrollment packet
• Assist patients with access resources, including quality of care, childcare services, food, housing, transportation, utilities (focusing on the SDoH Fields)
• Meet patient’s bedside in hospital or outreach where patients are located and conduct needs assessment using Social Determinants of Health Tool via EPIC
