We are seeking a Part-Time/PRN Physical Therapist (PT) to join our home health team in DuPage County, IL, Specifically Elmwood Park, Roselle, Schaumburg, Medinah, Bloomingdale, Itasca, Glen Ellyn, Addison, Hanover Park, and Streamwood. • Perform in-home physical therapy evaluations and treatments
• Develop and implement individualized rehabilitation plans of care
• Monitor patient progress and adjust treatment plans as needed
• Educate patients and caregivers on exercises, mobility, and safety
• Document visits accurately and efficiently in accordance with agency guidelines
• Communicate patient progress with the interdisciplinary care team

• Licensed Physical Therapist (PT)
• Doctorate in Physical Therapy (DPT)
• Home Health experience preferred, but not required
• Strong communication and documentation skills
