Job Summary

We are seeking a compassionate and organized Registered Nurse (RN) to serve as a Home Health Case Manager. This role is responsible for coordinating and delivering skilled nursing care to patients in their homes, developing and managing individualized plans of care, and ensuring compliance with physician orders, payer requirements, and state regulations.

Key Responsibilities

• Perform comprehensive initial, ongoing, and discharge assessments
• Develop, implement, and update patient-centered plans of care
• Provide skilled nursing services in the home as ordered by the physician
• Coordinate care with physicians, caregivers, aides, and interdisciplinary team members
• Conduct patient and caregiver education related to diagnoses, medications, and treatments
• Supervise and provide clinical oversight to LPNs, CNAs, and HHAs as applicable
• Ensure accurate, timely clinical documentation in the EMR
• Monitor patient progress and report significant changes to the physician
• Participate in quality improvement, case conferences, and compliance activities
• Follow all agency policies, HIPAA requirements, and state/federal regulations

Qualifications

• Active Registered Nurse (RN) license in the State of Indiana (or applicable state)
• Minimum 1 year nursing experience (home health experience preferred)
• Strong assessment, care coordination, and clinical judgment skills
• Ability to work independently and manage a caseload effectively
• Valid driver’s license, reliable transportation, and current auto insurance
• Current CPR certification

Preferred Experience

• Home health, hospice, or case management experience
• Experience with OASIS and EMR systems
• Knowledge of Medicare/Medicaid home health regulations
