About This Position
REPORTING RELATIONSHIP
Supervised by: Practice Manager
Interrelationships: Care team members, contracted facilities, care partners, patients, patient families, and other staff members.
JOB SUMMARY
To serve as the company’s scheduling coordinator of the patient visits by providers, nurses, and medical assistants.
Essential Functions:
1. Schedule visits of all patients on Dynamic Wound Care’s patient roster.
2. Have a good understanding of care partners preferences for schedule of visits.
3. Communicate with care providers on finalization of schedule and any changes made in a timely manner.
4. Have a good understanding of patients’ needs and take into consideration when creating the visit schedule.
5. Daily calls to confirm the next days’ visit for each patient and documenting in the patient’s chart.
6. Observes confidentiality and safeguards all patient related information.
7. Accepts responsibility for regular attendance and punctuality and fulfills job requirements during scheduled working hours.
8. Communicate with field staff on any changes (day-of) to the schedule.
9. Field incoming calls regarding questions or concerns pertaining to the visit schedule of patients.
10. Track patient status, communicating known hospitalizations, deaths, travel etc. to the clinical team.
11. Assist in answering phones in the office when needed.
12. Update and maintain the patient master list.
13. Update and maintain the patient hospitalization list.
14. Call the hospital list of patients to see if they have been discharged or know of date of discharge.
15. Work the referral log and place on the schedule promptly
16. Send weekly schedules to facilities, home health, via fax, text message, or email – to be completed by Thursday at 3pm in order to make any necessary changes or corrections.
Requirements:QUALIFICATIONS
Education/Degree:
High school diploma or GED equivalent
Knowledge/Skills/Ability:
Ability to work independently, make accurate and at times, quick judgements. Ability to respond timely and appropriately to requests pertaining to the day-to-day schedule of patient visits. Acceptance of and adaptability to different social, racial, cultural, and religious modes.
Experience:
Minimum of 2 (two) years of experience in scheduling in a Healthcare setting required. Understanding of medical terminology, ability to multi-task, coordination of schedules with other agencies and facilities. Computer skills, customer service skills, ability to meet deadlines timely and ability to make last-minute necessary corrections or changes upon request.
Physical Requirements:
- Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
- Must be able to sit for long periods of time.
- Use of phone, keyboard, mouse, computer screens.
Mental Requirements:
- Must be able to work independently, make judgements based on assessments and data available and act accordingly.
- Must be flexible, innovative and possess good interpersonal skills.
- Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Transportation:
Must have a current valid driver’s license, auto liability insurance, and reliable transportation.
