The Supportive Services Coordinator assures access to Transportation and Educational Services so transportation is not a barrier to healthcare and supportive services. Transportation support includes medical, dental, behavioral health, and other health-related appointments. This role also provides educational services that promote client independence.
Key Responsibilities1. Eligibility Determination- Assess transportation needs and determine the most appropriate mode of transportation for each client.
- Process supportive services assessments while managing resources across multiple funding streams.
- Complete participant reassessments every 180 days per AIDS Institute Supportive Services standards.
- Educate participants and stakeholders on transportation access; assist with scheduling.
- Ensure compliance with transportation service policies and procedures.
- Provide timely follow‑up to transportation requests, calls, and inquiries.
- Travel to rural areas as needed to provide program services.
- Arrange educational support and transportation assistance for eligible participants using the most cost‑effective resources.
- Oversee distribution of bus passes, gas cards, cab rides, and other transportation resources.
- Network and collaborate with internal and external partners to achieve positive health outcomes.
- Schedule and participate in case conferences and reassessments.
- Support participants in developing independence and improved self‑management skills.
- Coordinate transportation needs across all Trillium Health departments.
- Educate clients on alternative transportation resources, including MAS (Medical Answering Service).
- Troubleshoot issues including complaints, incident reports, vendor billing, and identification of new vendors.
- Provide direct transportation using agency or personal vehicle.
- Document each transportation encounter as a progress note in chronological order in the client chart.
- Maintain participant records including attendance, participation, progress, referrals, and additional needs.
- Complete electronic documentation in the EHR and data entry in the AIDS Institute AIRS system.
- Track transportation resource inventory and request replenishments in coordination with Finance.
- Maintain spreadsheets/databases for transportation resources and due dates for assessments.
- Verify attendance at allowable health‑related and HIV‑related medical appointments.
- Document all educational services provided.
- Assist with required monthly program reporting.
- Bachelor’s degree in Health & Human Services, Social Work, or related field and 1 year of related experience; OR
- Associate degree in a related field and 2 years of related experience; OR
- High school diploma and 3 years of related experience.
- Knowledge of local and regional transportation systems.
- Must have a reliable vehicle and meet agency driving requirements, including a valid NYS driver’s license and automobile insurance.
While performing the duties of this job, the employee is required to stand, sit, walk, use hands, reach, talk, and hear. Occasional stooping, bending, and lifting/moving up to 25 lbs may be required. Vision requirements include close, distance, peripheral, and depth perception, with the ability to adjust focus.
