Position Summary

Heritage Behavioral Health Center is seeking a full-time Licensed Practical Nurse (LPN) to provide primary and psychiatric nursing services to individuals diagnosed with serious mental illness and chronic health conditions.

This role blends clinic-based psychiatric support and community-based nursing care, serving clients in outpatient settings, group homes, and client residences.

Schedule
  • Full-Time (40 hours per week)
  • Monday through Friday, 8:00 AM – 5:00 PM, every other Friday off paid for wellness
Core Responsibilities
  • Provide direct nursing support to a psychiatric provider in the DeWitt County clinic two days per week, including patient assessments, medication administration, documentation, and coordination of care.
  • Deliver community-based nursing services three days per week in group homes and client residences, including well-being checks, basic nursing assessments, medication education, and administration of long-acting injectables.
  • Obtain and accurately document vital signs, physical observations, and health-related concerns across all settings.
  • Administer prescribed medications as ordered; monitor and report adverse reactions promptly.
  • Provide medication monitoring and education to promote safe and effective use.
  • Monitor client health status and ensure timely, accurate, and complete documentation in the electronic health record.
  • Communicate clearly with providers regarding changes in condition, urgent needs, or follow-up care.
  • Build positive, respectful relationships with clients, families, and residential staff.
  • Maintain organization and availability of nursing supplies and medications across sites.
  • Coordinate with pharmacies to ensure timely refills and medication availability.
  • Facilitate referrals and linkage to medical and behavioral health services as needed.
  • Collaborate with multidisciplinary team members to support urgent medical needs and triage care appropriately.

Education and Experience Requirements
  • Active, undisciplined Licensed Practical Nurse (LPN) license in the State of Illinois
  • Excellent organizational skills and attention to detail
  • Ability to work independently and as part of a multidisciplinary team
  • Compassionate, empathetic approach with a commitment to patient-centered care
Compensation

Hourly Range: $25.00 – $34.00, dependent on education and experience
