About This Position

Our Hospice is looking for a Patient Care Manager for our Columbus Clinical team. This position will be responsible for ensuring the delivery of exceptional, patient‑centered hospice care. This role provides leadership and clinical oversight to nursing and support staff, maintains compliance with state and federal regulations, and collaborates closely with interdisciplinary teams, referral sources, and community partners. The Patient Care Manager plays a vital role in maintaining operational excellence, quality outcomes, and an environment of continuous improvement.

Key Responsibilities: Leadership & Team Support
  • Serve as a role model demonstrating professionalism, integrity, and teamwork.
  • Provide leadership to clinical staff, hold team members accountable, and support staff development.
  • Participate actively in Clinical Leadership Team meetings and organizational committees.
  • Assist with hiring, onboarding, evaluations, and staff engagement initiatives.
Clinical Oversight
  • Act as a clinical resource to team members, admissions staff, and interdisciplinary groups.
  • Ensure timely responses to patient, family, and partner inquiries.
  • Oversee coordination of patient care 24/7, ensuring high‑quality and cost‑effective service delivery.
  • Ensure accurate documentation for certifications, re-certifications, and care level changes.
Operational & Quality Management
  • Maintain knowledge of hospice and palliative care regulations and ensure compliance.
  • Promote continuous quality improvement, investigate concerns, and participate in Performance Improvement Plans.
  • Review quality metrics and contribute to the development of strategies to achieve organizational goals.
  • Manage patient care expenses including staffing, supplies, pharmacy, and equipment to remain within budget.
Communication & Collaboration
  • Facilitate effective communication among interdisciplinary team members, physicians, vendors, and caregivers.
  • Keep staff informed of organizational changes, policy updates, and operational expectations.
  • Practice a participative leadership style that values staff input and feedback.
Technology
  • Maintain strong proficiency in the electronic medical record and incorporate technology into care processes.

Qualifications
Education
  • Bachelor's Degree in Nursing from an accredited University.
  • Hospice or palliative care certification preferred.
Experience
  • 3–5 years of healthcare or hospice experience preferred.
  • Experience managing patient care teams or a 24/7 nursing unit strongly preferred.
  • Proven leadership experience.
Licensure & Certifications
  • Valid Indiana Registered Nurse (RN) license.
  • Valid Indiana Driver’s License.
  • CPR Certification.
Skills
  • Strong leadership, communication, interpersonal, and motivational skills.
  • Excellent problem‑solving abilities.
  • Knowledge of healthcare regulations and hospice clinical standards.
  • Proficiency with Microsoft Word, Excel, PowerPoint, and EMR systems.
Working Conditions
  • Primarily office‑based with some travel or outside exposure.
  • Must be able to lift up to 10 lbs occasionally and perform frequent walking, standing, and reaching.
  • Must follow universal precautions; potential exposure to bloodborne pathogens (Category C).
