As the Wellness Director, you serve as the clinical and operational leader of resident care services, ensuring excellence in care delivery, regulatory compliance, staff development, and resident well-being.

Job Title: Wellness Director

Reports To: Executive Director

FLSA Status: Exempt

Position Summary

This position is responsible for overseeing all wellness and care services within the community, including staff supervision, clinical oversight, medication management, care coordination, regulatory compliance, and quality improvement initiatives.

The Wellness Director collaborates closely with leadership, families, healthcare providers, and regulatory agencies to ensure safe, compliant, and resident-centered care.

Essential Duties and Responsibilities

Core duties and responsibilities include, but are not limited to, the following:

Clinical & Wellness Oversight

  • Oversee resident assessments, service plans, and ongoing care coordination
  • Ensure safe and accurate medication management and administration practices
  • Collaborate with physicians, pharmacies, and healthcare providers
  • Monitor resident health status and respond appropriately to changes in condition

Staff Leadership & Development

  • Supervise, schedule, train, and evaluate wellness staff including CNAs, Resident Assistants, and wellness support roles
  • Ensure staff competency, certification, and ongoing training compliance
  • Provide coaching, corrective action, and performance feedback as needed
  • Promote teamwork, accountability, and a positive care culture

Regulatory Compliance & Documentation

  • Maintain compliance with DHS Chapter 83 and Chapter 89 regulations
  • Ensure accurate and timely documentation in electronic health record systems
  • Prepare for and participate in surveys, audits, and inspections
  • Maintain policies, procedures, and care standards

Quality Assurance & Risk Management

  • Monitor incident reports, falls, medication errors, and care trends
  • Lead corrective action plans and quality improvement initiatives
  • Promote infection control, safety, and emergency preparedness practices

Resident & Family Communication

  • Communicate with residents and families regarding care needs and concerns
  • Participate in care conferences and service planning meetings
  • Promote resident satisfaction and service recovery when needed

Administrative & Operational Support

  • Assist with budget awareness, staffing efficiency, and resource management
  • Collaborate with leadership on census support and operational planning
  • Participate in leadership meetings and interdisciplinary collaboration

General Responsibilities

  • Serve as on-call clinical support as required
  • Perform additional duties as assigned by the Executive Director or leadership team

Supervisory Responsibilities

The Wellness Director provides direct supervision to wellness and caregiving staff and participates in hiring, training, performance evaluations, and disciplinary actions.

Requirements:

Qualifications

Education and Experience

  • Previous leadership or supervisory experience required
  • Experience in assisted living, senior living, or healthcare settings strongly preferred

Certificates, Licenses, and Training

  • CBRF Training in First Aid/Choking, Fire Safety, and Standard Precautions training required
  • Knowledge of assisted living regulations required
  • Completion of state approved CBRF Administrator Course within 2 years of accepting position

Schedule and Availability

  • Full-time position with flexible hours based on community needs
  • On-call availability required, including evenings, weekends, and holidays
