Job Description

Serves as a member of a multidisciplinary team responsible for conducting psychiatric assessments; assessing physical health needs; making appropriate referrals to community physicians; providing management and administration of medication in conjunction with the psychiatrist; providing a range of treatment, rehabilitation, and support services. . This position can, with supervisor approval, be considered for remote work
1. Provide community support for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team; assume primary responsibility for developing the person centered plan.
2. Conduct comprehensive assessment of psychiatric history, mental status, and diagnosis; physical health and dental health.
3. Provide medication administration and medical services and arrange for required lab work according to protocol.
4. Under the direction of team psychiatrist – organize and manage the system of getting medication to clients and integrating medication administration and manage pharmaceuticals and medical supplies.
5. In collaboration with team psychiatrist coordinate, schedule and administer medical assessments of client physical health, making appropriate referrals to community physicians for further assessment and treatment, and coordinate psychiatric treatment with medical treatment.
6. Provide on-call crisis intervention and face-to-face availability as needed after hours and participate in on-call schedule.
7. Document client progress according to agency procedures: dictation notes to be submitted by noon the day after services provided - if not then responsible for completion of notes by agency determined standard.
8. Provide direct clinical services to clients on an individual, group and family basis in the office and in the community settings to teach behavioral symptom management techniques to alleviate and manage symptoms.
9. Provide practical help and supports, advocacy, coordination, side-by-side individualized support, problem solving,direct assistance, training and supervision to help clients obtain the necessities of daily living and provide individual supportive therapy, social-skill development and assertiveness training to increase client social and interpersonal activities in community settings.
10. Responsible for attending mandatory trainings and remaining in compliance with all mandatory trainings requirements including, but not limited to, NCI, CPR, First Aid and Division required trainings for the delivery of ACTT services.
11. Responsible for completion of authorizations and all accompanying paperwork for ACTT services for assigned consumers with a 95% minimum compliance rate.
12. Participate in daily staff or organizational meetings and person centered planning meetings.
13. Provide specific number of hours of direct services to consumers each month that satisfy productivity requirements.
14. Establish and maintain positive and effective working relationship with CVH colleagues, including being respectful of colleagues’ ideas, contributing to team processes, using constructive problem-solving processes, communicating proactively and efficiently, and maintaining an overall positive workplace atmosphere.
15. Documentation shall maintain compliance with CVH policies/procedures, purchaser of service requirements (i.e. NC Division of MH/DD/SA, DMA, LME), accrediting/regulatory bodies (CARF, NC Statues, APSM rules), and the NC Records Manual.
16. Participate in and effectively use clinical and administrative supervision to improve the quality and performance of services for person served and the organization.
17. Respect and preserve the privacy, confidentiality and security of all confidential information.
18. Always promote, protect, and advocate for the rights and diversity of persons served, and continually protect persons served from abuse, neglect, and exploitation. 19. Establish and maintain effective therapeutic and professional relationships with persons served, family members, legally responsible persons, or others significant to the person’s treatment and support, within recognized and approved emerging/promising/best practices of service modalities.
20. Other duties as assigned or appointed.

Preferred and Minimum Position Qualifications
1. Must meet the requirement of a RN with a QP status according to 10A NCAC 27.G0104 or be an Advanced Practice Nurse according to NCGS Chapter 90, Article 1, Subchapter 32M.

2. 1-2 years experience with SPMI population.

3. Knowledge/skill in the use of computer hardware/software, e.g.,Microsoft products (Word, Excel, Acess, and Powerpoint), web research, etc.
