Job Title: Registered Nurse (RN) Discharge Planner (Hagerstown)

Position Summary
Talantage is seeking a dedicated Registered Nurse (RN) Discharge Planner to join our team at our client's correctional institution. The RN Discharge Planner is responsible for coordinating comprehensive discharge planning within a correctional healthcare setting to ensure continuity of care following patient release.

Working under the direction of the Director of Nursing, the RN Discharge Planner collaborates with a multidisciplinary team to develop and implement safe, effective, and individualized discharge plans.

Key Responsibilities
• Coordinate discharge planning for patients transitioning from a correctional setting to the community
• Assess patient medical, behavioral health, and social needs related to discharge
• Collaborate with nursing, medical providers, behavioral health staff, and community agencies
• Arrange follow-up care, medication access, and referrals to community resources
• Ensure discharge plans promote continuity of care and patient safety
• Maintain accurate and timely documentation in accordance with policies and regulations
• Comply with all facility security requirements and correctional healthcare standards

Qualifications
• Current Registered Nurse (RN) license in the state of Maryland or ability to obtain licensure
• Current BLS and CPR certifications
• One (1) year of experience as a Discharge Planner or Case Manager required
• Experience in correctional healthcare or acute care preferred
• Ability to obtain and maintain security clearance, including drug screening and criminal background check
• Strong communication, organizational, and care coordination skills
