We are currently seeking an experienced:

HOSPICE ADMISSION NURSE - RN

Full Time, Days

• Evaluate patients for admission.
• Develop plans of care for patients admitted with the approval of the certifying physician.
• Coordinate patient care service as necessary among various disciplines and Clinical Managers.
• Document patient care data as required.
• Position is primarily field based, with extensive computer and telephone use. Position involves extensive travel, mostly intra-state. Patient care-70%; Travel-20%; Office time-10%

• Requires current RN licensure within state of branch location with at least two years of hospice clinical nursing experience.
• Successfully completed nursing education from an approved school of nursing.
• Able to provide effective health care services to patients in their care environment through patient evaluation (comprehensive assessment), care, planning and coordination. This includes thorough, accurate and complete documentation of the visit.
• Must accept travel and working conditions as assigned. Must be able to work independently and possess strong communication, organization and time management skills.
• Must be able to use a computer and have a working knowledge of a variety of computer applications such as Windows, Word, etc. CPR certification is required.
• Minimum of twelve (12) hours continuing education annually.
