Center for Life Management (CLM) provides Southern New Hampshire’s most comprehensive array of outpatient mental health services. The Medical Transcriptionist is responsible for accurately and efficiently transcribing medical and behavioral health documentation dictated by healthcare providers. This role requires exceptional attention to detail, strong command of language, and strict adherence to confidentiality standards to ensure high-quality, timely, and compliant clinical documentation that supports client care.

This position is fully remote following the completion of an onsite training period, typically expected to last approximately two weeks. Training duration may vary depending on a candidate’s learning pace and proficiency with CLM’s transcription processes and systems. This is a full time, 40 hours per week position.

Major Functional Areas and Description of Duties

Transcription & Documentation

  • Accurately and efficiently transcribe medical and behavioral health reports dictated by physicians, nurse practitioners, clinicians, and other healthcare professionals.
  • Transcribe a variety of documents including progress notes, therapy notes, psychiatric evaluations, chart reviews, treatment summaries, and other clinical documentation.
  • Complete all assigned transcriptions within three (3) business days of receiving dictations, while maintaining accuracy and adherence to documentation standards.
  • Apply proper grammar, punctuation, and sentence structure consistent with medical transcription best practices.
  • Conduct research as needed to verify terminology, provider names, medications, and diagnostic information.
  • Edit and format completed transcriptions for accuracy, clarity, and compliance with clinical standards.
  • Return completed reports to designated providers for review, signature, or correction within required turnaround times.

Quality, Compliance & Confidentiality

  • Ensure transcriptions are error-free, grammatically correct, and aligned with CLM documentation standards.
  • Maintain strict compliance with confidentiality requirements, including HIPAA and all legal and regulatory standards.
  • Identify discrepancies or missing information and flag them for follow-up or clarification.
  • Support quality improvement initiatives related to documentation accuracy and workflow optimization.

Technology & Workflow Management

  • Operate transcription equipment and software, including dictation systems, word processing programs, and other required applications.
  • Troubleshoot minor equipment or software issues as needed.
  • Manage daily tasks independently and meet assigned deadlines with minimal supervision.
  • Participate in training sessions and maintain proficiency as systems, templates, and documentation requirements evolve.

General Responsibilities

  • Perform functions professionally and in a manner that protects the integrity and confidentiality of clients and staff.
  • Actively participate in appropriate team meetings and committees as requested by supervisor.
  • Represent CLM in a professional and appropriate manner in all settings.
  • Perform all other duties as assigned by supervisor.

Physical Demands and Working Conditions

The majority of the work takes place in remotely after the initial onsite training period. Manual dexterity is required to use computers, transcription equipment, and standard office tools. The ability to sit for extended periods of time while performing transcription is required. Reliable high-speed internet and a private, secure workspace suitable for confidential work are required.

Requirements:

Qualifications and Skills

  • At least 3–5 years of medical transcription experience in a healthcare setting required. Completion of a certificate program in medical transcription, healthcare documentation, or a related field may be considered in lieu of some experience.
  • Previous experience transcribing in a medical, psychiatric, or behavioral health setting preferred.
  • Training or coursework in anatomy, medical terminology, diagnostic procedures, and healthcare documentation standards is strongly preferred.
  • Proficient transcription skills with a high level of attention to detail and accuracy.
  • Strong command of the English language, including grammar, spelling, punctuation, and structure.
  • Excellent listening skills and the ability to discern dictation clearly, including variations in speech patterns, accents, and clinical terminology.
  • Strong understanding of medical terminology, anatomy, pharmacology, and behavioral health documentation standards.
  • Ability to work independently, manage time effectively, and meet productivity and turnaround expectations.
  • Strong computer skills, including proficiency with Microsoft Office and transcription software.
  • Ability to learn quickly, follow detailed specifications, and adapt to new templates, systems, and requirements.
  • Prior remote work experience in a documentation or administrative support role preferred.
  • Reliability, accountability, and strong communication skills.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
