WHY THIS ROLE MATTERS
This is a dual-function role that sits at the heart of how we serve our participants every day. As a Life Enrichment and In-Home Transition Specialist, you will be the bridge between participants, their families, caregivers, and our care coordination team — ensuring that every person we serve receives high-quality, person-centered care both at our adult day center and within their home. You will combine life enrichment coordination with in-home waiver compliance, caregiver training, and incident follow-through to help participants remain safely in their homes and communities.
WORK LOCATION & SCHEDULE
• This is a fully onsite position based at our Lexington, Kentucky office — five days per week, Monday through Friday
• Candidates are not required to live in Lexington, but must be able to reliably commute to the Lexington office daily
• Field travel is required — daily and weekly visits to participant homes and community locations within approximately a two-hour radius of the Lexington office
• Occasional flexibility for early evening calls or home visits based on participant and caregiver needs
WHAT YOU WILL DO
• Serve as the primary relationship bridge between participants and the Golden Years care coordination team
• Explain care plans, service expectations, and participant rights in clear, respectful language and assist in resolving routine questions and concerns
• Support person-centered planning by ensuring participant goals, preferences, and safety needs are reflected in the plan of care
• Provide emotional support and companionship to participants, promoting overall well-being and social engagement
Life Enrichment Coordination
• Coordinate and schedule care services for participants including transportation, medical appointments, and necessary treatments and therapies
• Conduct comprehensive assessments of participants' physical, mental, and emotional health to determine care needs
• Develop and maintain individualized care plans in collaboration with participants and their families
• Help participants engage in social activities and maintain meaningful relationships with friends and family
• Own and manage the schedule board for caregivers and participants, ensuring efficient and accurate coordination of care services
Caregiver Transition & Training
• Lead caregiver transitions into the home including first-day introductions, review of expectations, and warm hand-off from the care coordinator
• Provide structured, waiver-aligned caregiver training on authorized tasks including personal care, safety, infection control, fall prevention, documentation, and professional boundaries
• Reinforce caregiver understanding of incident reporting expectations, confidentiality, and respect for participant choice and dignity
• Monitor caregiver placement and performance during the early phase of assignment and communicate needed changes or additional training to the care coordinator and leadership
In-Home Assessments & Compliance
• Conduct scheduled and unannounced home visits to validate that services align with the HCB waiver plan of care including tasks, hours, and safety needs
• Complete phone-based assessments — both announced and unannounced — to verify caregiver presence, service delivery, and participant well-being
• Observe and document the home environment for risks and strengths using Golden Years tools and Medicaid-aligned standards
• Monitor and remind participants of medication regimens as appropriate within non-medical scope
Incident Reporting & Quality Follow-Up
• Educate caregivers and participants on what constitutes a reportable incident and how to report it appropriately
• Receive, document, and follow through on incidents including falls, medication errors, and suspected abuse or neglect in accordance with Golden Years and Medicaid/HCB timelines
• Track follow-up actions, communicate with the care coordinator and leadership, and support corrective actions to prevent recurrence
Documentation & Reporting
• Maintain accurate and up-to-date participant records including care plans, progress notes, visit documentation, and incident reports
• Generate reports as required by regulatory agencies and participate in team meetings to discuss participant progress and care coordination
• Provide guidance, training, and performance support to care staff — ensuring adherence to established care plans and protocols
WHAT WE ARE LOOKING FOR — MUST HAVES
• Experience in home and community-based services, caregiving, case management, life enrichment, or a similar role — Medicaid waiver or HCBS experience strongly preferred
• Bilingual candidates are strongly encouraged to apply — this is a significant plus for this role
– Swahili-speaking candidates are especially sought and will be given strong preference
– Arabic, French, and Spanish are also highly valued
– Proficiency in any additional language beyond English will strengthen your application
• Reliable transportation, valid driver's license, and current auto insurance for daily and weekly travel within a two-hour radius of the Lexington office
• Strong communication skills with the ability to build trust and maintain professional boundaries with participants, caregivers, and external partners
• Demonstrated ability to coach, train, and support caregivers — including giving clear feedback and reinforcing expectations
• Solid documentation skills and attention to detail for plans of care, visit notes, training records, and incident reports
• Comfort with frequent in-home visits, field travel, and phone assessments both announced and unannounced
• Alignment with the Golden Rule and Golden Years values — respect, kindness, empathy, and genuine commitment to community enrichment
NICE TO HAVE — NOT REQUIRED
• Experience with Kentucky HCB Waiver programs, participant-directed services, or other HCBS waivers
• Background in social work, human services, nursing assistant, activities or life enrichment, or quality and incident management
• Familiarity with electronic visit verification (EVV) and basic EMR or waiver documentation systems
• Personal caregiving experience — candidates with lived caregiving backgrounds are strongly encouraged to apply
COMPENSATION, BENEFITS & GROWTH
• Starting pay: $19.00 – $25.00 per hour based on experience, certifications, bilingual skills, and role fit
• All new team members complete a 90-day probationary period to confirm reliability, cultural alignment, and role fit before transitioning to regular employment status
• Paid training and hands-on onboarding with experienced team members in HCB waiver expectations, caregiver selection, training methods, incident reporting, and Golden Years policies
• 6-month performance review and annual reviews thereafter with clear KPIs tied to documentation quality, caregiver training completion, timely incident reporting, and participant and caregiver feedback
