Licensed Practical Nurse (LPN) – MHHS Rural Health ClinicPosition Summary

The Licensed Practical Nurse (LPN) at our Rural Health Clinic provides high‑quality, patient‑centered nursing care in an outpatient primary care environment. Working under the supervision of a Registered Nurse and/or Provider, the LPN supports clinical operations, patient flow, and care coordination to ensure exceptional service for our rural community.

Essential Duties & Responsibilities
  • Provide direct patient care in accordance with nursing standards, clinic protocols, and provider instructions.
  • Obtain and document vital signs, medical history, chief complaint, medication lists, and other intake information.
  • Assist providers with examinations, procedures, wound care, immunizations, point‑of‑care testing, and diagnostic collection (e.g., urine, strep, flu, COVID‑19 swabs).
  • Administer medications per provider orders, including oral medications and injections.
  • Perform clinical tasks such as EKGs, nebulizer treatments, pulse oximetry, specimen handling, and CLIA‑waived testing.
  • Support chronic care management by providing patient education, medication reconciliation, and follow‑up outreach.
  • Maintain accurate, timely documentation in the electronic health record (EHR).
  • Participate in care coordination, referrals, and communication with specialists, hospitals, and ancillary services.
  • Reinforce infection control, safety practices, and compliance with MHHS policies, HIPAA, and regulatory guidelines.
  • Maintain exam rooms, supplies, and medical equipment to ensure cleanliness, readiness, and proper functioning.
  • Support clinic operations by assisting with triage, scheduling needs, patient phone calls, and workflow optimization.
Qualifications
  • Education: Graduate of an accredited Practical Nursing program.
  • Licensure: Current, unrestricted LPN license in the State of West Virginia.
  • Certifications: BLS/CPR certification required (or obtained within 30 days of hire).
  • Experience:
    • Clinic, family practice, internal medicine, or ambulatory care experience preferred.
    • New graduates are encouraged to apply.
  • Skills:
    • Strong assessment, communication, and clinical documentation skills.
    • Ability to multi‑task in a fast-paced rural clinic environment.
    • Proficiency with computers and electronic health records (Epic, Athena, or similar).
    • Demonstrated patient‑centered approach and excellent customer service.

Preferred Qualifications:

  • Experience working in a rural health clinic.
  • Previous collaboration experience with Nurse Practitioners or other advanced practice providers.
  • Additional certifications related to wound care or pain management.
  • Familiarity with electronic health record (EHR) systems for documentation.
  • Continuing education in patient education methodologies or nursing best practices.

Responsibilities:

  • Provide comprehensive wound care management, including assessment, dressing changes, and monitoring for signs of infection.
  • Perform venipuncture and other routine nursing procedures safely and accurately.
  • Educate patients and their families on health conditions, treatment plans, and preventive care measures.
  • Administer pain management interventions and monitor patient responses to ensure comfort.
  • Document patient care activities thoroughly and communicate effectively with Nurse Practitioners and interdisciplinary teams.
  • Assist in daily patient care activities within an assisted living facility, ensuring adherence to established protocols and standards.
  • Monitor patient vital signs and report any significant changes to the supervising healthcare provider.

Skills:

The required skills such as wound care, patient education, and venipuncture are applied daily to provide direct and effective nursing care, ensuring patient safety and recovery. Proficiency in English enables clear communication with patients, families, and healthcare team members, facilitating accurate documentation and education. Nursing license credentials validate the candidate's ability to perform clinical tasks competently and within legal scope. Pain management skills are essential to assess and alleviate patient discomfort, improving overall care quality. Preferred skills, including experience in assisted living facilities and collaboration with Nurse Practitioners, enhance the nurse's ability to integrate into multidisciplinary teams and adapt to specialized care environments.
