Essential Functions:

• Complete Start of Care (SOC), admissions, and resumption of care visits
• Perform comprehensive physical, functional, and psychosocial assessments
• Develop individualized plans of care aligned with physician orders and Medicare guidelines
• Complete accurate and timely OASIS documentation
• Coordinate care with physicians, therapists, and interdisciplinary team members
• Identify needs for additional services, equipment, and community resources
• Educate patients and families to ensure a smooth transition into home health
• Support care planning from admission through discharge
• Participate in case conferences and quality improvement initiatives

• Strong clinical assessment and critical thinking skills
• Experience with OASIS and home health documentation
• Ability to work independently and manage a flexible schedule
• Excellent communication and care coordination skills
• Organized, reliable, and patient-focused

• Active, unrestricted Registered Nurse (RN) license in the state of Colorado; graduate of an accredited nursing program (BSN preferred).
• 2+ years of RN experience preferred; home health and OASIS experience strongly preferred.
• Current CPR certification required.
• Valid driver’s license, reliable transportation, and auto insurance.
• Knowledge of home health regulations, OASIS, and nursing assessment/care planning.
• Ability to maintain HIPAA compliance and adhere to all regulatory and agency standards.
• Completion of required TB screening and health documentation.
