Job Summary:

The Community Inclusion Case Manager is responsible for coordinating, monitoring, and advocating for individuals with intellectual and developmental disabilities enrolled in the Supports for Community Living (SCL) and Michelle P. Waiver (MPW) programs. This role is focused on helping individuals achieve independence and integration within their local communities through person-centered planning, advocacy, and collaboration with a variety of stakeholders.

Key Responsibilities:

  • Person-Centered Planning: Develop and implement personalized care plans for clients, ensuring that their goals align with their desires for community involvement, personal growth, and independence.
  • Service Coordination: Identify, connect, and monitor community-based services that align with each individual's needs and preferences, including social, recreational, educational, and vocational activities.
  • Advocacy: Serve as a liaison between clients, families, and service providers, ensuring that the rights and preferences of individuals are respected and upheld in all settings.
  • Community Integration: Promote and facilitate opportunities for individuals to engage meaningfully with their communities, including social and cultural events, group activities, and volunteer opportunities.
  • Monitoring and Compliance: Regularly review and assess the quality and effectiveness of services provided to ensure they meet both state and federal regulations and individual goals.
  • Documentation and Reporting: Maintain accurate, up-to-date client records, documenting service plans, progress notes, and any changes to care, as required by regulatory and funding agencies.
  • Collaboration: Work in partnership with a multidisciplinary team—including healthcare providers, social workers, and family members—to ensure comprehensive care plans and community inclusion strategies are effectively implemented.

Qualifications:

  • Education: Bachelor's degree in a human services field such as social work, psychology, or special education is required.
  • Experience: Previous experience in case management, community services, or working with individuals with intellectual and developmental disabilities is preferred.

Skills:

  • Strong communication skills for building relationships with clients, families, and service providers.
  • Excellent organizational skills to manage multiple clients and services.
  • Problem-solving and critical thinking to address challenges related to community integration and service delivery.
  • Ability to work independently and as part of a collaborative team.
  • Certifications: Completion of required training specific to SCL and MPW services as mandated by the Kentucky Department for Medicaid Services.

Working Conditions:

  • Travel: Regular travel within the assigned service area to meet with clients, families, and service providers.
  • Schedule: Full-time position with flexible hours, including occasional evenings and weekends as needed to accommodate client schedules.
