Objective / Purpose

Under the direction of the Program Manager, the Service Coordinator is responsible for developing, implementing, coordinating, and monitoring services for individuals with intellectual and developmental disabilities. This role ensures that each person’s needs and goals are met through a person-centered approach.

Major Responsibilities1. Service Coordination
  • Facilitate and document the person-centered planning process, including development, implementation, and management of the Individual Service Agreement (ISA).
  • Complete all required documentation accurately and on time.
  • Assist individuals in identifying strengths, needs, and goals; maintain regular interaction with assigned clients.
  • Review ISA expectations, objectives, and data collection systems to ensure consistent implementation and compliance.
  • Coordinate services with other providers (e.g., physicians, specialists, therapists, psychologists) as needed.
  • Conduct regular face-to-face visits in the individual’s home and community, including at least one home visit per month.
  • Ensure adherence to all state regulations.
2. Program Coordination & Development
  • Assess individual needs and make appropriate service recommendations.
  • Ensure all waiver services are scheduled and delivered as required.
  • Collaborate with clinical team members to develop individualized services.
  • Ensure goals, objectives, and record-keeping systems are accurate and implemented promptly.
  • Serve as liaison between agency personnel and external providers during service development.
  • Regularly evaluate program effectiveness and communicate with service providers as needed.
3. Client Advocacy
  • Communicate with staff, agency programs, and other human service agencies to exchange relevant information.
  • Refer individuals to appropriate services and maintain ongoing contact with providers.
  • Develop resources and essential support services as needed.
  • Advocate on behalf of individuals and monitor the quality of services provided.
4. Administrative & Networking
  • Respond promptly to requests for information and documentation.
  • Promote the agency’s reputation and cultivate support for its services.
  • Represent UCS professionally to the public and referral sources.
  • Serve as an agency representative as assigned.


Qualifications:
Qualifications
Education & Experience
  • Required: High School Diploma or equivalent
    • Plus relevant Service Coordination experience OR
    • At least one year of experience in Developmental Services
  • Preferred: Bachelor’s degree in Human Services or a related field
Skills
  • Proficiency in Windows applications, including typing, navigation, and email usage
  • Strong interpersonal, communication, and writing skills
  • Ability to manage documentation accurately and meet deadlines
Licenses & Certifications
  • Valid Driver’s License
