The Patient Navigator is a member of the multi-disciplinary outpatient behavioral health team. The Navigator serves as a resource and liaison between patients, practitioners, clinical and administrative staff. The Navigator will provide short-term case management with patients to identify their barriers to healthcare and connect them to the resources they may need including, but not limited to, state financial assistance, insurance, counseling, and/or transportation. The Navigator is vital in providing proactive attention to patients to optimize and secure connections to on-going treatment by promoting the continuum of care and connecting to resources both internal to UMass and external as needed.

I. Major Responsibilities:

1. Provide case management and referrals to meet patient needs

2. Build community relationships with outside referral sources

3. Attendance to various staff meetings in accordance with requirements and to ensure program staff involvement.

4. Provide reliable transportation for patients as needed

5. Connecting patients to the UMass Harrington Healthcare Behavioral Health continuum of care

6. Providing follow-up contact and resources for patients

Standard Staffing Level Responsibilities:

1. Complies with established departmental policies, procedures, and objectives.

2. Attends variety of meetings, conferences, seminars as required or directed.

3. Demonstrates use of Quality Improvement in daily operations.

4. Complies with all health and safety regulations and requirements.

5. Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to creating and maintaining an environment of professionalism, tolerance, civility and acceptance toward all employees, patients and visitors.

6. Maintains regular, reliable, and predictable attendance.

7. Performs other similar and related duties as required or directed.

II. Position Qualifications:

License/Certification/Education:

Preferred:

1. Bachelor’s Degree.

Experience/Skills:

Required:

1. Excellent communication skills and interpersonal skills.

2. Valid driver’s license and proof of insurance

3. Strong organizational skills

4. Willingness to work with a multi-disciplinary team.

Preferred:

1. Two years working within the mental health or human services field.

2. Experience working directly with patients affected by substance use disorders and mental health disorders preferred or willingness to learn.

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Department-specific competencies and their measurements will be developed and maintained in the individual departments. The competencies will be maintained and attached to the departmental job description. Responsible managers will review competencies with position incumbents.
