Join Christie Ossington Neighbourhood Centre (CONC) as a Housing Follow-Up Support Worker and help people achieve long-term stability by providing mobile case management, life skills support, and connections to vital community resources. In this role, you’ll walk alongside clients as they rebuild their lives with dignity and independence.
Position Summary
This full time Contract position provides mobile case management services for previously homeless clients to enable them to stabilize their housing situation and live independently. The Housing Follow-up Support Worker works as an important member of our Breaking Barriers Program Team. The position is scheduled Monday to Friday from 9am to 5pm.
In this role, you will complete intake and initial assessments for community members facing multiple barriers including experiences of homelessness, physical and mental health challenges, substance use, social isolation, histories of abuse and trauma, immigration, and histories within the criminal justice system. You will collaborate with our Life Management, Health & Wellness, Income Security and Sustainable Housing Teams to develop and coordinate effective case plans for shelter clients. The successful candidate for this role is highly motivated to support vulnerable people in truly transforming their lives. This role reports to the Supervisor of Housing Follow Up Program. This is a unionized position.
Key Responsibilities
- Complete intake with clients, including providing a thorough description of program offerings, and completing required documentation, with respect and compassion.
- Ensure integration and optimization of client-centered supports between the Life Management, Health & Wellness, Income Security and Sustainable Housing Teams. Schedule appointments and support clients’ progress through program offerings.
- Carry out home visits to clients who are at significant risk of homeless
- Provide case management planning, supportive counselling, crisis intervention, and information and referral services
- Advocate on behalf of clients with Landlords, OW, ODSP, and other services
- Enable clients to establish or maintain links with appropriate services including services related to general health care, addiction treatment, mental health care, identification/document replacement, volunteer opportunities, job training, and job placement
- Provide life skills training in areas including landlord-tenant rights and responsibilities, budgeting, housekeeping, and pet care
- Track outcomes and inform the inform the further development of Housing First Follow-up Support Service models
- Provide monthly service reports to the Supervisor of Housing Follow Up Support Program
- Provide relevant resources and referrals. Identify and facilitate access to clinical care, rehab, detox, and other support services.
- Participate actively in team meetings and contribute to problem-solving, progress reporting, documentation and performance planning, to achieve the highest standards of program delivery.
- Support cross-departmental collaboration, contribute with enthusiasm to agency initiatives, and embody the values of CONC as part of our unique communities. Contribute to a positive working environment.
Required Qualifications
- Proven experience and success in housing homeless clients. Passion for the important work of supporting vulnerable individuals in their pursuit of improved health and well-being, including knowledge/expertise in our program areas- Life Management, Health & Wellness, Income Security and Sustainable Housing.
- Demonstrated professional experience working as part of a team.
- Minimum 2 years delivering client-centered support services
- Minimum 2 years directly supporting vulnerable community members in a shelter or similar setting
- College Diploma or University Degree in a related field, or equivalent combination of education in a health-related environment.
- Demonstrated experience in research and program development.
- Demonstrated understanding of strategies required to work effectively with vulnerable populations, including identifying assets and barriers
- Experience working with Indigenous communities
- Knowledge of Shelter Standards and conflict resolution strategies. First-Aid, WHIMIS and CPI, Mental Health and Wellness, Trauma-informed care, Harm reduction, System Navigation, case management and working with vulnerable and marginalized individuals with complex needs, and history of homelessness is required.
- Familiar with community resources including substance abuse programs and other support services for homeless communities in Toronto
- Demonstrated ability to build collegial and effective professional relationships with community members, staff teams, partner organizations and the general public
- Skilled in organization of work, time management, excellent oral and written communication including MS Office
- Experience administering Ontario Common Assessment of Needs is an asset
- Presentation of a current criminal record check
