Responsibilities:

• Provide direct patient care in accordance with hospice care plans.

• Collaborate with physicians, social workers, and other healthcare professionals.

• Conduct patient assessments and create personalized care plans.

• Administer medications and treatments as prescribed.

• Educate patients and families about care plans and symptom management.

• Document patient care activities accurately and timely.

Qualifications:

• Valid RN license in the state of Texas.

• Minimum of 1-2 years of nursing experience (hospice experience preferred).

• Strong clinical and critical thinking skills.

• Excellent communication and interpersonal skills.

• Ability to work independently and as part of a team.
