Segue Inc. is a highly regarded non-profit outpatient mental health facility with a legacy of over three decades dedicated to delivering exceptional mental health services to the residents of Jackson and Hillsdale counties in Michigan. Position: Mental Health Case Manager

As a Mental Health Case Manager, you will play a pivotal role in our DREAMS Case Management program. Your responsibilities will involve assisting individuals with mental health challenges in accessing vital services, including entitlements, medical care, social support, education, and more. Your advocacy and support will empower individuals to achieve their highest level of recovery and functioning.

Schedule: Monday through Friday, 8:00 AM to 4:30 PM

Requirements for Consideration:

• Verifiable Bachelor's degree in Social Work, Human Services, Criminal Justice, or Nursing (or licensed Registered Nurse - RN)
• Qualified Mental Health Professional (QMHP) with specialized training or a minimum of one year of experience working with individuals with mental illness
• Proficient typing skills (minimum 40 WPM)
• Proficiency in using iPads, Apple cell phones, and other technologies preferred
• Effective multitasking abilities in a fast-paced environment
• Strong organizational and time management skills
• Excellent communication and teamwork abilities
• Previous experience with Electronic Medical Records (EMR) system preferred, but not required
• Valid driver's license, vehicle insurance, and reliable transportation required
