Grief doesn't follow a schedule—and neither does healing. At Lifted Hospice, our Bereavement Coordinators walk with families through their journey of loss, offering presence, support, and resources long after the patient’s passing.

This is a role of deep compassion and consistent follow-through. You provide counseling, guidance, and meaningful outreach to those navigating grief, while ensuring Lifted meets or exceeds regulatory and ethical bereavement care standards. You also serve as a steady presence for the interdisciplinary team, helping align bereavement services with each family’s needs, culture, and experience.

Your Role in the Mission

As a Bereavement Coordinator, you provide and coordinate support for grieving families across our service area. You assess risk, offer counseling or referrals, and maintain ongoing communication throughout the 13-month bereavement period. You are also responsible for maintaining documentation, supporting community outreach, and collaborating with the IDT to ensure bereavement care is personalized, appropriate, and documented per CHAP standards.

Whether you’re hosting a memorial, sending a handwritten note, or helping a family connect with counseling resources, your role helps Lifted extend dignity, love, and support—beyond the bedside.

Key Responsibilities

  • Assess bereavement needs – Conduct timely, compassionate grief assessments to identify risk level, cultural needs, and appropriate support pathways for families and caregivers.
  • Provide grief support – Offer individualized grief counseling or emotional support to family members through in-person, phone, or written communication.
  • Coordinate ongoing bereavement outreach – Manage and deliver appropriate follow-up contact (calls, letters, emails, or visits) across the 13-month bereavement timeline, in alignment with CHAP and Lifted standards.
  • Document all support activities – Maintain accurate, timely records of assessments, outreach, and follow-up within the EMR to ensure CHAP-compliant bereavement documentation.
  • Refer to appropriate resources – Identify and refer families to licensed counselors, community services, or faith-based organizations as needed.
  • Participate in IDT – Collaborate with the interdisciplinary group/team to share updates, coordinate support, and ensure families' needs are integrated into the full plan of care.
  • Support memorial events and mailings – Assist with planning and facilitating community or agency-led remembrance events, grief groups, and seasonal mailings.
  • Track program outcomes – Monitor bereavement activities and share insights with leadership to support continuous improvement and regulatory alignment.
  • Stay current and prepared – Maintain up-to-date knowledge of grief theory, bereavement best practices, and eligibility standards for public and private support programs.
  • Represent Lifted with compassion – Embody professionalism, cultural sensitivity, and kindness in every interaction—especially in emotionally sensitive moments.
Skills & Qualifications
  • High school diploma or equivalent required; bachelor's degree in social work, psychology, counseling, or a related field preferred.
  • Experience in grief counseling, hospice bereavement, or a similar emotional support role is required.
  • Working knowledge of Medicare, Medicaid, and private insurance programs related to hospice bereavement support is preferred.
  • Experience with EMR documentation; Kantime system experience preferred.
  • Excellent interpersonal and communication skills, including verbal support and sensitive written communication.
  • Ability to remain calm, compassionate, and grounded in emotionally intense or high-risk family interactions.
  • Knowledge of local community counseling, spiritual, or social support resources.
  • Strong time management and follow-through in both autonomous and team settings.
  • Must be able to read, write, and communicate clearly in English.
  • Valid driver’s license, reliable transportation, and valid auto liability insurance.
Where You’ll Work

This is a primarily in-office role, based at a local Lifted Hospice location. You’ll provide bereavement care and follow-up through phone calls, letters, resource coordination, and EMR documentation. While most support will be provided from the office, you may occasionally attend memorial events, interdisciplinary meetings, or in-person visits as needed to support families and community engagement.

This role requires professionalism, emotional presence, and strong follow-through in a structured, team-based environment.
