About This Position
PRINCIPAL ACCOUNTABILITIES OF POSITION
Primarily responsible for providing training, care, and assistance to persons with intellectual and development disabilities.
ROLE OF THE LIFE SKILLS PROGRAM INSTRUCTOR
The Life Skills Program Instructor is primarily responsible for providing training, care, and assistance to individuals with intellectual and developmental disabilities (I/DD). The Instructor supports each Individual in achieving their highest level of independence through a person-centered approach, ensuring services are delivered in alignment with individualized goals, service plans, and organizational standards.
ESSENTIAL FUNCTIONS
1. Instructor
- Establish and maintain supportive, respectful, and professional relationships with Individuals, their families, and collateral contacts.
- Provide individual-specific service in compliance with service descriptions as defined in service directory/specific attachments and core assurances.
- Create and maintain an atmosphere of warmth, dignity, personal interest, and positive engagement within the program environment.
- Review and understand each Individual’s behavioral support plan and specific needs.
- Observe, recognize, and document changes in an individual’s condition; seek appropriate intervention and report concerns promptly to the Program Manager or Director.
- Assist Individuals with meals, including food preparation as needed.
- Assist Individuals with personal hygiene, including toileting and diapering.
- Greet, assist, and escort Individuals to their classrooms and all areas within the facility. Assist with daily dismissal from the program.
- Assist Individuals with mobility, including use of adaptive or self-help devices as instructed.
- Facilitate or assist in a minimum of two (2) classes each day, adhering to person-centered focus philosophy.
- Accompany Individuals to a minimum of two (2) community outings per week.
- Monitor the health and safety of all Individuals at all times.
- Monitor IP goals for effectiveness at least annually, but as needed, and report to the Program Manager.
- Administer medication if medication certified.
- Attend support plan and IP program meetings as possible.
2. Administration
- Complete all assigned documentation requirements in a timely manner as required, including daily notes/service logs and summaries on progress towards established support plan and implementation plan goals.
- Submit required documentation and timesheets by the business day following the end of each reporting period.
- Immediately notify the supervisor of any incidents, accidents, concerns, or operational difficulties.
- Prepare General Event Reports using BOM of FL documentation system and submit to required parties by end of business day.
- Enter complete notes daily into BOM of FL selected system.
ADDITIONAL DUTIES
- Assist Individuals with boarding and disembarking from transportation.
- Serve as a positive role model for adults in the program through appropriate dress, speech, attitude, and courtesy.
- Plan and implement activities aligned with Individuals’ goals, enrichment needs, and physical activity objectives, utilizing provided curriculum and materials.
- Comply with all company policies and procedures.
- Maintain confidentiality of all protected health information in accordance with HIPAA regulations.
- Attend required trainings as mandated for Med-Waiver providers.
- Perform other duties and special projects as assigned.
SUPERVISORY RESPONSIBILITY
- No supervisory responsibility.
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS
- Primary activities are conducted within a well-lit, climate-controlled facility.
- Occasional trips to client/prospect locations may require outdoor travel and potential exposure to unpleasant weather and hazardous road conditions.
- Adequate mobility that requires frequent walking, standing, bending, stooping, kneeling, reaching (vertical and horizontal), and twisting for extended periods of time.
- Ability to regularly lift, carry, push, pull, position, and/or transfer up to 75 pounds.
- Interact and communicate constantly with all individuals encountered. Must be able to exchange accurate and detailed information in all situations.
- Maintain concentrated attention to safety, detail and accuracy for sustained periods of time.
- Must be able to provide proper hygiene assistance to any individual that requires help.
QUALIFICATIONS
- High School diploma or equivalent.
- One-year experience in a medical, psychiatric, nursing, or child/adult care setting or direct care with individuals with intellectual and/or developmental disabilities.
- Valid state driver’s license
- Must pass Level II Background screening
KNOWLEDGE AND SKILLS
- Knowledge of proper transfer techniques and body mechanics to transfer clients with appropriate assistance and/or equipment (transfer board, slip sheet, Hoyer lift) as needed.
- Exhibit strong interpersonal and communication skills.
- Ability and interest in working with persons with developmental disabilities and physical limitations.
- Ability to establish and maintain cooperative working relationships.
