Job Summary:

Case Managers work under the supervision of the Administrator and are responsible for coordinating care and services for residents in our assisted living community. This role ensures residents’ physical, emotional, and social needs are met through individualized service plans, strong collaboration, and adherence to all regulatory requirements. Case Managers serve as advocates, supporting each resident’s quality of life, safety, and dignity.

What You'll Do:

  • Coordinate resident care in collaboration with the interdisciplinary team, families, healthcare providers, and community resources
  • Develop, update, and maintain individualized service plans in accordance with regulatory requirements
  • Support the admissions process, including coordination, documentation, and resident onboarding
  • Participate in team meetings and care conferences to ensure high-quality, person-centered care
  • Maintain accurate, timely, and compliant documentation, including care plans and case management notes
  • Conduct chart audits and support regulatory compliance with NYS Department of Health standards
  • Guide residents and families through care decisions, transitions, and available services
  • Facilitate discharge and transitions of care to ensure continuity and support
  • Coordinate access to benefits, healthcare services, and community resources
  • Collaborate with direct care staff to promote consistency and quality of care
  • Assist with audits, surveys, incident reporting, and quality improvement efforts
  • Uphold resident rights, dignity, privacy, and confidentiality
  • Build positive relationships with residents, families, and community partners

Compensation: $29.00-32.00/hr; commensurate with experience and credentials.

Schedule Considerations: Full time 40 hrs/wk (Monday-Friday 8:00a-4:30pm typical).

Benefits:

  • Employees regularly scheduled to work 20+ hrs/wk are eligible for Leave Sharing and employer-paid Group Life and AD&D.

  • Must be at least 18 years of age;
  • Minimum associate degree required - must be in human services or service delivery from an accredited college or university with three years of full-time experience in the provision of services to a dependent or frail adult population;
  • Bachelor's or Master's degree preferred - must be in social work, human services, or service delivery from an accredited college or university with one year of full-time experience in the provision of services to a dependent or frail adult population;
  • Must be able to communicate clearly, concisely, and with empathy to build rapport and achieve high quality of care;
  • Basic knowledge of resident rights;
  • Satisfactory completion of post-offer, pre-employment physical required;
  • Must meet eligibility requirements for approval by the NYS DOH.
