This role plays a key part in supporting patient care coordination within a fast-paced healthcare environment, ensuring seamless communication between patients, providers, and clinical systems. You will act as a central point of contact for patient needs, managing escalations, follow-ups, and care plan adherence while helping close gaps in care. The position involves both administrative and clinical support responsibilities, including EHR management, lab coordination, and referral tracking. You will work closely with physicians and care teams to ensure accurate information flow and timely resolution of patient issues. In this highly patient-focused role, you will also contribute to process improvements, system optimization, and quality enhancements. - Serve as the primary contact for patient escalations, resolving issues independently while ensuring timely follow-up and resolution.
- Support clinical intake processes, including patient screenings, documentation, and accurate data entry at the point of service.
- Manage electronic health records (EHR), including lab entry, referrals, requisitions, and provider communications.
- Coordinate patient care tasks, ensuring alignment with care plans and closing gaps through proactive outreach.
- Facilitate communication between patients, providers, and stakeholders to ensure continuity of care and accurate information flow.
- Resolve order clarifications, troubleshoot patient-related issues, and update pharmacy or referral information as needed.
- Support audits, identify workflow trends, and contribute to system or process improvements to enhance care delivery.
Requirements:
- High school diploma or equivalent required.
- Certified Medical Assistant credential required (CMA, RMA, CCMA, NCMA, CMAC, or NRCMA).
- Strong knowledge of healthcare environments, medical terminology, and chronic disease management.
- Experience working with electronic health records (EHR) and basic computer systems.
- Strong patient advocacy skills with the ability to navigate complex healthcare systems.
- Excellent communication skills, including reading, writing, speaking, and comprehension in English.
- Ability to work independently, prioritize tasks, and remain effective in fast-paced or high-stress environments.
- Strong judgment, professionalism, and attention to detail in patient interactions and documentation.
