Job Title: Program Manager, Caregiver and Family Support Services

Reports To: Senior Director of Caregiver and Family Support Services

Department: Caregiver and Family Support Services

Position Summary:

The Program Manager, Caregiver and Family Support Services, is responsible for the daily operations of Caregiver Supports Services, including AFC, FCSP, and all related caregiver initiatives. The Program Manager supports the Senior Director in overseeing the quality, integrity, and compliance activities as directed by MassHealth and AGE program regulations and NCQA accreditation standards. The Program Manager also participates in outreach activities, promoting program growth.

Job Responsibilities and Performance Standards:

  • Maintain working knowledge of AFC and FCSP regulations, procedures, and contracts
  • Participate in management of AFC and FSCP programs, including attending management and team meetings, and overseeing daily program operations
  • Provide administrative individual and group supervision to program staff
  • Oversee AFC Administrative Assistant activities and provide direct supervision
  • Oversee AFC Caregiver training
  • Develop program procedures consistent with MassHealth and AGE regulations, and NCQA accreditation standards; and maintain policies and procedures manuals
  • Develop and implement quality assurance activities to monitor program compliance
  • Participate in Quality Improvement (QI) efforts including but not limited to accreditation-related QI activities, survey preparation, statistical analysis, audit preparation, and data management
  • Oversee incoming referrals and assessment team activities
  • Provide program coverage as needed, conducting home visits and other case management activities
  • Assist Senior Director to expand program census by developing and implementing an outreach/marketing strategic plan
  • Liaise with Fiscal department to resolve billing and insurance matters in a timely manner
  • Provide assistance to program staff on all databases and act as liaison between staff and IT personnel
  • Participate in and contribute to required department, agency and external meetings, including Massachusetts Council for Adult Foster Care (MCAFC) meetings and Massachusetts Family Caregiver Support Services
  • Ensure compliance with HIPAA regulations
  • Provide after-hour coverage for agency programs according to agency on-call policies and procedures
  • Perform other duties as assigned

Qualifications:

  • Bachelor’s Degree required (human service related field preferred), plus a minimum of 5 years related experience.
  • Knowledge of the home care system and long-term care issues.
  • Project management experience preferred.
  • Ability to work independently and collaboratively with diverse populations.
  • Exceptional time management and organizational proficiency.
  • Must meet both CORI/SORI and public health screening requirements.

  • Physical surroundings are comfortable with minimal exposure to injury or hazards.

Social/Psychological Conditions:

  • Occasional stress due to periodic or cyclical workload pressures and deadlines. Some interruptions involved.

Physical Effort:

  • Frequently sits, stands, walks, bends, reaches, and stoops throughout the day.
  • Frequently lifts, pulls, pushes, and carries up to 20 lbs.
  • Periodic eye strain and light ear strain.
