Position Summary: We are seeking a compassionate and reliable Physical Therapist Assistant (PTA) to provide skilled therapy services in patients’ homes under the supervision of a licensed Physical Therapist. In this role, you will help patients improve mobility, function, and overall quality of life by implementing individualized care plans.
What You'll Do:
- Deliver physical therapy treatments as directed by the supervising PT
- Perform therapeutic exercises, mobility training, and transfer assistance
- Utilize modalities such as heat, cold, electrical stimulation, and traction
- Adjust treatments within scope based on patient response
- Observe and document patient response to treatment
- Report changes in condition, pain levels, or safety concerns to the PT
- Collaborate with the care team to support positive outcomes
- Teach home exercise programs and safety techniques
- Support patients and caregivers in following the plan of care
- Complete accurate, timely documentation in EMR
- Ensure compliance with Medicare, Medicaid, and agency guidelines
- Participate in quality improvement initiatives
- Follow infection control and safety protocols in the home
- Maintain HIPAA compliance and patient confidentiality
- Attend trainings and meet continuing education requirements
- Associate degree from an accredited PTA program
- Active, unrestricted Colorado PTA license
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
Preferred:
- 1+ year of PTA experience
- Home health experience
