Job Summary:

The Admission Coordinator at Alpas Wellness La Plata is responsible for conducting thorough assessments for newly admitted patients, including gathering medical, psychological, and social history. They will complete dimension notes based on ASAM criteria and ensure accurate, compliant documentation in the electronic health record (EHR) system. This role involves collaborating with the clinical team to integrate care plans and provide patients with necessary support and orientation about the facility. The Admission Coordinator also acts as a point of contact for patients and their families, addressing concerns and explaining the treatment process.

Essential Duties and Responsibilities:

  • Conduct thorough admission assessments for all newly admitted patients.

  • Gather and document medical, psychological, and social history to ensure comprehensive understanding of the patient’s needs.

  • Complete detailed dimension notes per ASAM (American Society of Addiction Medicine) criteria, ensuring accurate and thorough documentation of each patient’s condition and treatment requirements.

  • Accurately input all necessary patient data into the facility's electronic health record (EHR) system.

  • Ensure all admission documentation complies with organizational policies, state regulations, and accreditation standards.

  • Coordinate with the clinical team to ensure seamless care planning and integration of services.

  • Act as a point of contact for patients and their families, addressing concerns and providing information about the admission process.

  • Provide compassionate support and education to patients, ensuring they understand the admission process and initial steps of their treatment plan.

  • Assist in orienting patients to the facility, including explaining rules, schedules, and available resources.

  • Collaborate with other departments to support operational needs and ensure the highest standard of care.

  • Collects and analyzes data related to departmental performance indicators.

  • Assists therapists and other senior program staff in crisis intervention as needed.

  • Communicates with medical staff, nursing service, and other department supervisors.

  • Attends all required training, in-service and staff meetings.

  • May be subject to the handling of and exposure to hazardous chemicals.

  • May be subject to exposure to infectious waste, diseases, conditions, etc, including TB, HIV, AIDS, and Hepatitis B viruses.

  • Maintain patient confidentiality, staff confidentiality; treat staff and patients with kindness, dignity and respect.

  • Comply with Patient’s Rights rules.

  • Other duties as assigned

Required Knowledge, Skills, and Experience:

  • High School diploma or GED preferred; associate or bachelor’s degree in Psychology, Social Work, Behavioral Health, or a related field preferred

  • Certification in addiction counseling or behavioral health (e.g., CAC-AD, CSC-AD, or comparable) is a plus

  • Knowledge of ASAM criteria strongly preferred

  • CPR/First Aid certification or willingness to obtain upon hire

  • If in recovery, a minimum one - year period of sustained sobriety required

  • Warm, compassionate and empathetic personality

  • Dignified character that models recovery principles

  • Understanding of substance use disorders, co-occurring mental health conditions, and the addiction recovery process

  • Working knowledge of the ASAM criteria and its application in determining levels of care

  • Familiarity with HIPAA, 42 CFR Part 2, and patient confidentiality laws

  • Experience with Electronic Health Records (EHR) systems (e.g., Kipu or similar platforms)

  • Awareness of state regulatory and Joint Commission standards related to admissions

  • Strong clinical interviewing and assessment skills

  • Excellent written and verbal communication skills, including the ability to document clearly and concisely

  • Ability to respond calmly and effectively in crisis situations

  • Strong interpersonal and customer service skills, especially in working with patients and families in distress

  • High attention to detail, accuracy, and compliance with regulatory documentation standards

  • Ability to multitask and prioritize in a fast-paced, high-stakes environment

  • Collaborative mindset with the ability to work effectively across departments

Schedule:

  • Part-time Weekends

Compensation:

  • $22 per hour + DOE
