HomeFirst Home Healthcare provides quality home health care to thousands of individuals in the state of Georgia. We are looking to hire an Intake Coordinator to join our Smyrna, Georgia home-health office team. This is a Monday thru Friday, on-site position.

Overview
The Intake Coordinator is responsible for accepting incoming calls for new referrals for clinical services provided to home care patients within an assigned geographical area.
- Receives clinical information on new patient referrals to the home health agency.
- Collects all data pertinent to the request for admission to include demographic data, medical history, medication, physical, discharge summary, labs, physician orders and insurance information
- Verifies insurance/payer’s source via DDE or specific system
- Ensures referral acceptance and communication within 15 minutes of receiving referral
- Notifies clinical designee responsible for scheduling and authorizations
- Contact patient/clinical designee prior to admission to ensure service location
- Liaisons between field, medical, office and referral source staff
- Obtains authorization from insurance companies and ensures physician licensure compliance
- Conducts follow up on current active patient hospitalizations and communicates with clinical supervisor to obtain orders as needed
- Maintains non-admit log
- Attends in-services and meetings
- High school diploma or GED equivalent
- Prior clinical home health experience preferred; physician practice, hospice experience a plus
- Knowledge of Medicare, TennCare, state and federal regulations, and accreditation standards as they relate to clinical home health
- Proficient in Microsoft Office Suite including Outlook, Work and Excel
- Familiarity with medical terminology
- Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
- Strong initiative, follow through, process orientation, attention-to-detail
- Demonstrated confidentiality, professional and business acumen
