The Opportunity: Care Coordinator, Hello House for Men

Job Type: Part- Time

Pay: Starting at $27/ hour

As Care Coordinator, you’ll have a dynamic and critical role working with individuals diagnosed with mental health, substance use, dual diagnosis, and co-occurring disorders to provide care management and support. You will support clients in accessing treatment services and community resources.

This role is about making a difference in the lives of the people we serve. As a Care Coordinator, you’ll bring the following qualifications:

  • A Bachelor’s Degree; Degree is desirable in psychology, social work, health care, administration, or related human service field.
  • Minimum of two years’ experience with social service organizations, and experience with coordination and referral for services.
  • Excellent communication skills, organization skills, and computer skills are required.

Care Coordinator Key Responsibilities:

  • Engage with clients following admission to help them access treatment services within VOAMASS and externally with community-based providers.
  • Facilitate 3 weekly groups with up to 16 residents on various topics.
  • Engage and build rapport with clients in order to assist with referrals and coordination for services including primary care, mental health treatment, addiction treatment, recovery support, employment specialists, housing specialists and other social supports.
  • Meet with each client biweekly to provide follow-up assessments and ensure that clients attend appointments and access treatment.
  • Establish and maintain good working relationship with community based providers to ensure continued coordination.
  • Serve as part of the integrated multidisciplinary care team to enhance client experiences and maintain agency compliance with state regulations and mental health professional standards.
  • Maintains records through our electronic record keeping system according to VOAMASS Policies and Procedures. Complete all documentation and authorizations promptly, and comply with professional standards as required.
