Summary:
The PMAD Center intake coordinator, under direct supervision of the PMAD Clinical Director, is a licensed professional trained in perinatal mental health who responds to the emotional needs of the perinatal mental health patients through phone triage, screening, assessment and crisis intervention if -eeded referring to the Clinical Supervisor, referral and outreach,
The intake coordinator also provides education, advocacy and support to secure treatment and services,
Duties & Responsibilities:
Administrative:
I ,
Provides administrative support including, but not limited to, the following:
o Responsible for organization and production of PMAD Center monthly calendar for patients with supervision & approval from Program Manager o Ensures brochures and material inventory levels at practice are maintained appropriately for New Patient, Loss and Graduating Patient folders,
o Responsible for recording daily huddle meeting notes & emailing out to team o Acts as PMAD liaison for hospital provided services & drs offices supplying them with materials and information about the PMAD program as needed,

Clinical Responsibilities:
Performs new patient telephone intake assessments in a calm, professional manner through interview with patients,
o provides prompt, efficient and empathetic responses,
Educates new patients on PMAD services available to them - individual therapy, group therapies, support groups and classes - as appropriate for each unique patient,
Refer patients who do not meet PMAD program criteria to the appropriate mental or medical health facility personnel, or specialized community service,
Maintain and complete EMR patient documentation according to department policies,
Act as supportive role during support group sessions with primary facilitator and when required facilitates support group when primary facilitator is unavailable,
Fields medication questions from patients and helps educates patients on relevant medications and side effects prescribed by PMAD prescriber,
Refills medications for active patients under the designation of and as ordered by PMAD Center prescribers,
o Functions as a collaborative member of the PMAD Center mental health team,
o Understand the cultural basis of client's health beliefs, illness behaviors and attitude toward and acceptance of treatment,
Participates in meetings, conference and workshops in the field of Maternal Mental Health,
Respects and maintains privacy and dignity of patients and assures patient confidentiality at all times

Education and Experience:

Bachelor degree in nursing or social work and currently licensed to practice as a Registered Nurse in the state of New Jersey,
PMH-C certification,

Minimum 2 years of working in the mental health field,
and/or with the perinatal population, preferably maternal mental health,
3,
Knowledge of medical terminology
4,
Knowledge and proficiency with Microsoft Word and Outlook
Knowledge, Skills and Abilities:
Practice Management and Electronic medical records software knowledge Ability to work independently
Strong oral and written communication skills are essential
Attention to accuracy and detail are essential
Ability to follow exact instructions from superiors
Demonstrates a clear understanding of the importance of teamwork and staff satisfaction
