About This Position
We are seeking a dedicated, uplifting, and compassionate Case Manager to join our team at Hope Gospel Mission. Position Objective: interview and connect people to Hope Gospel Mission programming, provide needs and goals assessments, create a renewal plan, and the assist Residents in navigating through this plan to success.
Requirements and preferences for this position:
- Mature Christian in agreement with Hope Gospel Mission’s faith statement
- Strong interpersonal and critical thinking skills
- Strong gift of discernment
- Ability to navigate crisis in a calm and professional manner
- Computer literate
- Current driver’s license and ability to meet approval requirements of HGM’s insurance company for driving mission vehicles
- Associate degree or equivalent experience in related field preferred
- Experience with case management, addiction recovery, or social services is preferred
Summary of responsibilities:
- Create and maintain an intentional, faith-based, healthy, healing community
- Participate in outreach activities to promote programming options to potential residents and referral sources.
- Process incoming program applications, conduct structured intake interviews, and provide necessary referrals to applicants that need alternative services.
- Gather necessary data to complete and maintain accurate resident records.
- Work with each assigned resident to assess their needs and goals and then collaboratively develop an individualized renewal plan that outlines steps, resources, and timelines. (Academics, finances, life skills, nutrition/fitness, parenting, spirituality, vocational, other as needed)
- Assist residents with scheduling, navigating their program, accomplishing goals on their renewal plan, and connecting with necessary resources. Collaborate with other staff as necessary. (Chaplain, renewal counselor, house staff, vocational training staff, external support, etc.)
- Use motivational interviewing techniques to encourage engagement and persistence in reaching goals.
- Document all significant activities, events, interactions, and observations.
- Share in the responsibility to lead classes or groups as necessary. (Finances, life skills, nutrition and fitness, parenting skills, vocational skills, etc.)
- Assist with GEDs, academics, budgeting, resumes, job applications, etc. as necessary
- Meet regularly with each resident on case load to provide support and accountability.
- Celebrate each resident’s goal achievements by recognizing them at our monthly Celebration Dinner.
Case Load Capacity = 12 men and 6 women (women with children count as 3)

Typical hours are Monday – Friday, 8:00 to 5:00. Because we are a 24 hour / 7 day per week ministry, there are times when a resident may have an urgent need or question outside of those hours. Also, our Celebration Dinner is in the evening on the last Friday of each month.
