JOB SUMMARY AND QUALIFICATIONS:
The Home Coordinator, with the direction from the Residential Service Coordinator, will be responsible for the daily operations of 2 supported living homes for people with intellectual and developmental disabilities. This includes supervising and training a team of Direct Support Professionals in those homes.
WHAT YOU MUST HAVE:
This position requires a strong passion to see our clients excel and lead full and productive lives. This position requires the following:
- At least 2 years of experience working with adults who have intellectual and developmental disabilities.
- Knowledge of applicable regulatory requirements
- Planning and organizational skills
- Skills in good judgement, supervising, and training staff
- Ability to communicate effectively in a courteous and professional manner
- Ability to maintain confidentiality and uphold HIPAA
- Ability to work as a team member
To excel in this role the Home Coordinator will be strategic, inclusive, collaborative, opportunity-minded, self-motivated, adaptable, effective at building relationships, and motivated to make change happen for individuals who have IDD. Additionally, excellent communication and interpersonal skills are a must.
RESPONSIBILITIES:
- Maintain safety and cleanliness of the home, while following VOA policies and procedures
- Supervise and assist consumers in maintaining wholesome and productive lives with the emphasis on assuring that opportunities are provided for the consumers to achieve that which they envision for their own lives
- Train all Direct Support Staff per mandated curriculum as required by state and VOA policies and regulations
- Accompany consumers on all scheduled and emergency medical appointments/consultations, and ensure communication with the agency's Registered Nurse
- Attend all Interdisciplinary team meetings as assigned by the Residential Service Coordinator
- Complete all weekly, monthly, and quarterly documentation to assure compliance with PCISP goals and State Regulations
- Train all staff on each client's Individualized Support Plan and ensure annual training courses are completed as well
- Plan and coordinate leisure, recreational, and social activities
- Assist consumers in maintaining relationships with family and friends
- Responsible for the supervision of staff and assigned customers. Must be available on an on-call basis
- Develop staff schedules with budgetary constraints and guidelines to provide staff ratios based on consumer needs and approved cost plans
- Review on an ongoing basis hours worked and authorized pay by approving timesheets for their assigned Direct Support Staff
- Reconcile scheduled time off and actual time worked by each staff member. Participate in weekly overtime evaluations
- Monitor and evaluate staff performance
- Recommend disciplinary action for staff not meeting satisfactory performance or violation of policies
- Performance and quality improvement duties as assigned by Residential Service Coordinator
- Initiate purchase requests in a timely manner to meet the needs of the consumers we serve
- Ensure that grocery/supply shopping is done appropriately and within the client's budgets
- Ensure the maintenance on the homes vehicles is being completed
- Ensure MARS are accurate and up-to-date
- Communicate with the agency's Registered Nurse on any health concerns for the consumers
- Ensure that all medical appointments and encounters are documented in the Practical Health System
