About This Position

Shift Schedule: Monday-Friday 9AM-5PM

Hours Per Week: 35


The Program Director will be responsible for overseeing and implementing JASA’s Geriatrics, Alzheimer’s and Dementia Related Diseases (ADRD) programming for GWEP activities. The position will circulate among Montefiore sites, including FQHCs and the Center for the Aging Brain, and will work as part of an inter-disciplinary to address the clinical and social needs of older adults, and patients living with dementia and their caregivers.


The Program Director will:


  • Be an expert in community resources and social supports for geriatric and ADRD clinicians, patients and their caregivers.

  • Serve as a key member of an inter-disciplinary geriatrics/dementia care team and as a bridge for clinicians, patients and caregivers to community resources.

  • Deliver caregiver support and education to reduce burden for dyads.

  • Provide direct social support to geriatric patients, individuals living with ADRD and their caregivers by addressing the social determinants of health and addressing comprehensive social needs.

  • Create comprehensive care plans with patients and caregivers utilizing targeted assessments to identify needs and preferences for care. Utilize a person-centered approach to identify what matters most to dyads and connect them to services that align with their goals, maintain quality of life and enable stable home-based functioning.

  • Provide continuous support to clients across settings with direct services and referrals that may include: respite care, caregiver support and education, home modifications and safety, food and nutrition support, transportation, housing, residential care facilities, legal/financial planning, LTC planning, advance directives.

  • Plan ongoing meetings, calls and home visits with patients and caregivers to evaluate functioning, monitor risk, assess the status of current services, and update care plans; seeking appropriate guidance from the inter-disciplinary team in situations requiring consultation.

  • Participate in cross-discipline team meetings with healthcare providers.

  • Train other providers on geriatric/ADRD social supports and community resources.

  • Complete required case records, reports, and statistics.

  • Ongoing meetings and calls with clients to evaluate client functioning, monitor risk, assess the status of current services, and update and revise the case plan; seeking appropriate guidance from the care team in situations requiring clarification and consultation.

  • Identify and engage community members to participate in advisory board meetings.

  • Screen and match patients and caregivers with appropriate clinical trials.


QUALIFICATIONS:


  • A master’s level graduate social work professional with prior experience in ADRD care or case management and/or supervisory experience.

  • Demonstrated organizational skills and ability to provide supervision and leadership.

  • Understanding and appreciation of the roles of race, religion, ethnicity, sexual preference, and individual values as they relate to serving clients and families.

  • Ability to read, write and speak Spanish preferred.
