Job Overview:

Provides direct clinical social work services to patients and their families in a multidisciplinary team approach. Completes an initial psycho-social assessment including social work treatment plan. Provides individual, family or group counseling, complex discharge planning, bio ethics case consultation, referrals to community agencies and entitlement programs. Ensures continuity of care for those patients with complex post-acute care needs and maximizes the patient s and family s abilities to meet the continuing care needs within their social and financial framework. Understands stages of growth and development to assess each patient s age specific needs to provide age specific treatment and care. Documents interventions in patient s chart and submits monthly statistical information to the department.

Qualifications:

Required:

• Graduation from an accredited graduate school of social work with an MSW master s in social work degree
• State licensure required by the NJ State board of social work examiners
• Licensed social worker LSW or licensed clinical social worker LCSW depending upon years of social work experience and clinical course work
