About This Position

Licensed Practical Nurse (LPN)

Transitions At Home, is currently seeking to hire a Full-Time Licensed Practical/Vocational Nurse (LPN) in the Rice Lake area.

What does a Transitions at Home LPN do?
Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient's physician orders and plan of care under the direction and supervision of the RN Clinical Manager.
Hours: Full-Time
  • 40 hours per week 8:00 am - 4:30pm
  • Monday through Friday
  • No Weekends or Holidays
  • Limited on-call rotation

Qualifications / Job Requirements:
  • Active Wisconsin LPN License
  • Two (2) years' nursing experience, 1 year in Home Health (preferred)
  • CPR Certification
  • A valid Wisconsin Driver's License and Car Insurance
