Position Overview:

The PSH Case Manager Specialist is responsible for overseeing a team of case managers, providing supervision, training, and quality assurance to ensure effective service delivery. The PSH Case Manager Specialist will assist with complex cases and may hold a case-load of clients with higher needs.

Essential Duties and Responsibilities:

  • Guide, mentor, support and train a team of case managers under the direction of the Stabilization and Supportive Housing Specialist.
  • Review client files, and documentation ensuring compliance with program requirements.
  • Maintain various spreadsheets and tracking systems
  • Manage high-needs cases and provide expertise in complex situations.
  • Conduct regular supervision.
  • Communicate concerns or potential problems to Leadership in a timely manner.
  • Attend appropriate meetings and trainings as needed.
  • Work collaboratively with community partners
  • Perform all other duties as assigned by the Stabilization and Supportive Housing Specialist and/or the Director of Shelter Case Management.

Required Qualifications and Skills:

  • Minimum four (4) years of experience
  • Associate’s or Bachelor’s degree in Human Services, or related field
  • Minimum 7 years of human service experience.
  • Demonstrated leadership skills
  • Demonstrated case management skills.
  • Strong written and verbal communication skills.
  • Excellent interpersonal skills.
  • Proficiency with data reporting systems
  • Must be empathetic to the needs of a diverse socio-economic population.

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer
  • Must be able to lift up to 15 pounds at times

Work Environment:

  • This role is primarily required to be in person, with limited opportunities for hybrid work
  • Work must occur between Monday-Friday 8:00am-5:00pm
  • Must be able to travel in between work locations.

Compensation:

A salary within the range of $30.24 to $40.91 per hour will be provided to the successful candidate having met the minimum requirements for the position consistent with the MOC compensation philosophy.
