Respite Case Manager

VEVRAA Federal Contractor: Priority Referrals of Protected Veterans Requested

Under the direction of the Medical Respite Coordinator, the Case Manager Aide (CM Aide) provides supervision, safety, organization, and program management support to adult clients of the Medical Respite Program. Services are of limited duration; 30-60 days. The CM Aide networks with the greater New Haven shelters and social service system and collaborates with other Columbus House residential staff in order to provide an environment that reflects the core values of Trauma Informed Service: Safety, Trustworthiness, Choice, Collaboration and empowerment.

Qualifications, Knowledge, Skills and Abilities

  • High school diploma or equivalent required
  • Demonstrated ability to manage, motivate, and direct consumers
  • Knowledge of all aspects of delivering residential services for individuals experiencing homelessness
  • Strong organizational and time management skills with a deadline-oriented mindset
  • Proficiency in Microsoft Office and other common desktop applications
  • Ability to effectively utilize the local network of substance abuse, mental health, and social service providers
  • Familiarity with mental health, substance abuse, and dual diagnosis issues, as well as group dynamics
  • Knowledge of local housing resources and vocational/entitlement programs
  • Culturally competent and able to work effectively with diverse populations
  • Ability to work collaboratively within a team and foster teamwork
  • General understanding of crisis intervention, confidentiality, professional boundaries, and universal precautions
  • Ability to meet all documentation and reporting requirements
  • Ability to support and promote a Harm Reduction philosophy in the work environment
  • Strong empathy combined with high reliability and punctuality
  • Valid driver’s license with a good driving record

Responsibilities

  • Perform regular wellness rounds
  • Supervise up to 14 clients to ensure compliance with all house rules
  • Support and engage clients in following their recovery service plans
  • Observe and document medication calls
  • Assist with moving client belongings as needed
  • Clean and disinfect rooms after clients leave to ensure rooms are prepared for new admissions
  • Collaborate with other agencies and social service providers
  • Attend agency and program meetings as scheduled
  • Complete all required reports, including incident logs, case notes, and other documentation
  • Enforce house rules and maintain incident logs
  • Use the CT HMIS system and enter data as directed
  • Support the Program Coordinator by assisting with pre-screenings, intake/admissions, case notes, and discharges
  • Collaborate on the creation and implementation of individual service plans
  • Remain on-site to perform essential functions during emergency situations as designated by the Chief Executive Officer
  • Perform other related duties as assigned


Supervision: Medical Respite Recovery House Manager

Salary Range: $19 per hour

Schedule: Monday – Friday 4:00 p.m. to 12:00 a.m.
