LPN – Adult Foster Care (Full-Time / Part-Time)

Job Summary

We are seeking a compassionate Licensed Practical Nurse (LPN) to join our Adult Foster Care (AFC) team. This community-based role provides clinical oversight, member assessments, caregiver support, and health monitoring for adults and elders living safely at home.

This position offers flexibility, autonomy, and meaningful one-on-one member interaction.

Responsibilities

• Complete annual nursing assessments and clinical evaluations
• Develop and update AFC Plans of Care
• Monitor member health status and report changes to PCPs
• Conduct required home visits
• Review caregiver documentation and care logs
• Provide caregiver orientation and ongoing training
• Complete nursing progress notes and required reports
• Collaborate with Care Managers and interdisciplinary team

Qualifications

• Active MA LPN license
• CPR certified
• Minimum 2 years’ experience with elders or adults with disabilities
• Strong assessment and documentation skills
• Reliable transportation for community visits
