About This Position
Location: Remote role, but the individual must be located in Delaware County/Philadelphia area
The Assistant Program Manager (APM) supports the effective and efficient operation of nursing services by overseeing daily staff productivity, ensuring high-quality patient and client care, and promoting continuous improvement in healthcare delivery. This role works closely with clinical leadership and interdisciplinary teams to support operational excellence, staff development, and quality outcomes, with a primary focus on geriatric and palliative care populations. The APM is expected to travel regionally for site evaluations, staff support, and client engagement as needed.
Bachelor of Science in Nursing (BSN) required; Master of Science in Nursing (MSN) preferred
Minimum of three (3) years of experience in home health care, with a strong preference for geriatric and/or palliative care populations
Demonstrated clinical expertise in home health nursing and case management
Excellent verbal and written communication skills, with the ability to engage effectively with staff, patients, families, and stakeholders
Proven problem-solving abilities and strong interpersonal and relationship-building skills
Highly organized and detail-oriented, with the ability to manage multiple priorities and projects simultaneously
Willingness and ability to travel regionally, including the NYC area (White Plains and Long Island), for site visits and client meetings
Monitor daily productivity of nursing staff to ensure performance targets are met
Support optimization of scheduling and coverage to align staffing with patient needs
Review and manage flagged referrals in coordination with clinical leadership
Identify patients who would benefit from increased visit frequency to enhance outcomes through nurse case management
Audit nursing documentation for accuracy, completeness, and timeliness; ensure charting is completed within 24 hours
Oversee documentation of Chronic Care Management (CCM) minutes and resolve discrepancies
Review and assess cases exceeding 300 minutes of care to validate time justification and ensure documentation supports services provided
Provide day-to-day support to nursing staff and respond to operational and clinical workflow questions
Facilitate training initiatives to promote ongoing development and consistent practice standards
Support onboarding and orientation of new hires within their assigned territories, ensuring readiness and consistency across teams
Identify operational inefficiencies and recommend actionable solutions, including workflow and system improvements
Participate in leadership discussions, providing operational insights and field-based feedback
Support assigned projects and initiatives, including implementation tasks, follow-ups, and progress tracking
Conduct structured site visits to systematically evaluate nursing performance using standardized assessment tools and measurable performance indicators.
Perform daily monitoring and analysis of nursing schedules and visit completion data to ensure productivity benchmarks and staffing requirements are consistently met.
Compile, analyze, and report operational and performance metrics to senior leadership, providing timely, objective insights to support decision-making and continuous improvement.
